当飞利肝宁胶囊联合干扰素对慢性乙型肝炎肝纤维化血清SII、NLR、PLR、MLR水平的影响

摘　　要：目的探讨当飞利肝宁胶囊联合干扰素对慢性乙型肝炎肝纤维化患者血清系统免疫指数(SII)、中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、混合淋巴细胞培养反应(MLR)水平的影响。方法选取2018年7月—2020年7月在医院消化内科收治的慢性乙型肝炎肝纤维化患者126例,根据随机数字法,将126例患者分为两组各63例。对照组患者给予干扰素,观察组患者在对照组的基础上给予当飞利肝宁胶囊,两组患者均持治疗6个月。对比两组患者的临床疗效、全身乏力、食欲减退、腹胀、肝区疼痛等症状好转时间,对比两组患者的血清SII、NLR、PLR、MLR水平及血清炎性因子白细胞介素4(IL-4)、白细胞介素12(IL-12)、白细胞介素1β(IL-1β)水平。结果观察组临床疗效有效率高于对照组(P<0.05);观察组患者治疗后的全身乏力、食欲减退、腹胀、肝区疼痛好转时间均低于对照组(P<0.05);观察组治疗后肝功能谷丙转氨酶(ALT)、谷氨酸氨基转移酶(AST)、总胆红素(TBIL)水平低于对照组(P<0.05);观察组肝纤维化透明质酸酶(HA)、层粘连蛋白(LN)、IV型胶原(IV-C)、Ⅲ型前胶原(PCⅢ)水平低于对照组(P<0.05);观察组患者治疗后的血清SII、NLR、PLR、MLR水平均高于对照组(P<0.05);观察组患者血清炎性因子IL-4、IL-12、IL-1β水平低于对照组(P<0.05),观察组不良反应总发生率低于对照组(P<0.05)。结论当飞利肝宁胶囊与干扰素联合治疗慢性乙型肝炎肝纤维化,能改善患者肝功能及肝纤维化的程度,并能使血清SII、NLR、PLR、MLR水平及炎性因子水平达到有效降低,对机体肝脏恢复起到较好作用。Objective To investigate the effect of Dangfei Liganning Capsule(当飞利肝宁胶囊)combined with interferon on serum levels of SII,NLR,PLR and MLR in patients with chronic hepatitis B liver fibrosis.Methods A total of 126 patients with chronic hepatitis B liver fibrosis in the hospital from July 2018 to July 2020 were selected and randomly divided into two groups,63 cases in each group.The control group was given interferon and the observation group was given Dangfei Liganning Capsule on the basis of the control group.Both groups were treated for 6 months.The clinical efficacy,general fatigue,anorexia,abdominal distension,liver pain and other symptoms improvement time of the two groups were compared.The serum levels of SII,NLR,PLR,MLR and serum inflammatory factors IL-4,IL-12,IL-1βof the two groups were compared.Results The clinical effective rate of the observation group was higher than that of the control group(P<0.05).The improvement time of general fatigue,anorexia,abdominal distension and liver pain of the observation group after treatment were lower than those of the control group(P<0.05).The levels of ALT,AST and TBIL of liver function of the observation group after treatment were lower than those of the control group(P<0.05).The levels of HA,LN,IV-C and PCⅢof liver fibrosis of the observation group were lower than those of the control group(P<0.05).After treatment,the serum levels of SII,NLR,PLR and MLR in the observation group were higher than those in the control group(P<0.05).The serum levels of inflammatory factors IL-4,IL-12 and IL-1βin the observation group were lower than those in the control group(P<0.05),and the total incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Dangfei Liganning Capsule combined with interferon in the treatment of liver fibrosis in patients with chronic hepatitis B can improve the liver function and the degree of liver fibrosis,and can effectively reduce the levels of serum SII,NLR,PLR,MLR and inflamm
