谷青止鼾汤对阻塞性呼吸睡眠暂停低通气综合征的临床研究

摘　　要：目的分析谷青止鼾汤对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床疗效。方法采用前瞻性分析的研究方法,采用随机数字表法分为两组将2019年6月—2021年2月来医院进行治疗的72例OSAHS患者分为两组,各36例。两组均给予一般治疗,观察组给予中药颗粒剂谷青止鼾汤治疗,对照组设置对照,给予安慰剂治疗。对中医证候积分、各炎症因子水平、睡眠呼吸监测情况、各生命体征和嗜睡程度进行比较,并比较两组的临床疗效和不良反应。结果与治疗前相比,治疗后两组的各项中医证候积分、各炎症因子水平、睡眠呼吸暂停低通气指数(AHI)、氧减指数(ODI)水平、静息心率、呼吸次数、舒张压、收缩压、嗜睡评分均降低,且观察组较低,比较后差异有意义(P<0.05);治疗后两组的最低血氧饱和度(SaO_(2))、平均SaO_(2)水平均升高,且观察组较高;与对照组相比,观察组的临床总有效率(80.55%,29/36)明显较高,比较后差异有统计学意义(P<0.05);与对照组相比,观察组的头晕/头痛、恶心/呕吐、皮肤过敏、胃胀气、口舌干燥等发生率差异均无统计学意义(P>0.05)。结论谷青止鼾汤治疗OSAHS能够降低AHI、ODI,提高SaO_(2),缓解炎症反应及病情进展,改善临床症状和睡眠质量,且无明显不良反应,安全性较佳。Objective To analyze the clinical effect of Guqing Zhishou Decoction(谷青止鼾汤)on obstructive sleep apnea hypopnea syndrome(OSAHS).Methods Using a prospective analysis method,the random number table was used to divide the 72 OSAHS patients who came to the hospital for treatment from June 2018 to December 2020 into two groups,each with 36 cases.Both groups were given general treatment.The observation group was given Guqing Zhisuo Decoction,and the control group was given a placebo treatment.The scores of TCM syndromes,the levels of various inflammatory factors,the monitoring of sleep breathing,the vital signs and the degree of sleepiness were compared,and the clinical efficacy and adverse reactions of the two groups were compared.Results Compared with those before treatment,the scores of various TCM syndromes,levels of inflammatory factors,sleep apnea hypopnea index(AHI),oxygen hypoxia index(ODI),resting heart rate,respiratory frequency and diastolic blood pressure,the scores of systolic blood pressure and sleepiness of two groups were decreased,and those of the observation group were lower.The difference was significant after comparison(P<0.05).After treatment,the minimum blood oxygen saturation(SaO_(2))and average SaO_(2) levels of the two groups were increased,and those of the observation group were higher.Compared with that of the control group,the total clinical effective rate of the observation group was 80.55%(29/36),which was significantly higher(P<0.05).Compared with the control group,the incidence of dizziness/headache,nausea/vomiting,skin allergies,flatulence,and dry tongue of the observation group had no significant differences(P>0.05).Conclusions Guqing Zhisuo Decoction in the treatment of OSAHS can reduce AHI and ODI,increase SaO_(2),alleviate inflammation and disease progression.It can improve clinical symptoms and sleep quality,and has better safety with no obvious adverse reactions.
