二丹红藤败酱汤加味联合西医治疗湿热瘀结证盆腔炎性疾病后遗症疗效观察

摘　　要：目的观察二丹红藤败酱汤加味联合西医治疗湿热瘀结证盆腔炎性疾病后遗症(SPID)的临床疗效及对T淋巴细胞亚群和炎症因子的影响。方法选择2016年7月至2021年7月安徽省铜陵市中医医院妇科湿热瘀结证SPID住院患者80例,按照随机数字表法分为治疗组40例和对照组40例。对照组给予注射用头孢曲松钠联合替硝唑氯化钠注射液治疗,治疗组在对照组治疗基础上联合二丹红藤败酱汤加味治疗。治疗3个月经周期,比较2组临床疗效、中医证候评分、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、血清炎症因子[白细胞介素2(IL-2)、IL-6、IL-10、肿瘤坏死因子α(TNF-α)]水平变化。结果①治疗组总有效率95.00%(38/40),对照组总有效率80.00%(32/40),治疗组疗效优于对照组(χ^(2)=3.950,P<0.05)。②2组治疗后中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组治疗后中医证候各项评分及总分均低于对照组(P<0.05)。③2组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较本组治疗前升高,CD8^(+)较本组治疗前降低(P<0.05),且治疗组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于对照组,CD8^(+)低于对照组(P<0.05)。④2组治疗后血清IL-2、IL-10含量均较本组治疗前升高,IL-6、TNF-α含量均较本组治疗前降低(P<0.05),且治疗组治疗后血清IL-2、IL-10含量高于对照组,IL-6、TNF-α含量低于对照组(P<0.05)。结论二丹红藤败酱汤加味联合西医治疗能够改善湿热瘀结证SPID患者临床症状,提高治疗效果,可能与调节T淋巴细胞亚群、拮抗炎性反应有关。Objective To observe the clinical efficacy of modified Erdan Hongteng Baijiang Decoction combined with Western medicine in the treatment of sequelae of pelvic inflammatory disease(SPID)of damp-heat stasis syndrome and how to impact on T lymphocyte subsets and inflammatory factors.Methods A total of 80 SPID inpatients admitted to Department of Gynecology,Tongling Hospital of Traditional Chinese Medicine from July 2016 to July 2021 were randomly selected and divided into the treatment group and the control group,40 cases in each.The control group was treated with injection of ceftriaxone sodium combined with tinidazole sodium chloride,and the treatment group was additionally treated with Erdan Hongteng Baijiang Decoction on the basis of the control group.After 3 menstrual cycles,the clinical efficacy,TCM syndrome scores,T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)),serum inflammatory factors(interleukin-2[IL-2],interleukin-6[IL-6],interleukin-10[IL-10],tumor necrosis factor-α[TNF-α])were compared between groups.Results The overall effective rate in the treatment group was better than that of the control group(95.00%[38/40]vs 80.00%[32/40][χ^(2)=3.950,P<0.05],respectively).The scores of TCM syndromes in groups were lower(P<0.05),which in the treatment group was more common(P<0.05).CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in groups were higher than those before treatment(P<0.05),which in the treatment group were higher than the control group(P<0.05),while CD8^(+)in groups was lower(P<0.05),CD8^(+)in the treatment group was more prevalent(P<0.05).The levels of serum IL-2 and IL-10 in groups were higher(P<0.05),which in the treatment group were higher than the control group(P<0.05);while the levels of IL-6 and TNF-αwere lower than before treatment(P<0.05),which in the treatment group were lower than the control group(P<0.05).Conclusion For SPID patients(damp-heat stasis syndrome),modified Erdan Hongteng Baijiang Decoction combined with Western medicine can improve the clinical symptoms of and improve the ther
