自拟通便散外敷神阙治疗晚期肿瘤所致便秘的疗效研究

摘　　要：目的研究自拟通便散外敷神阙治疗晚期肿瘤所致便秘患者的临床效果。方法76例晚期肿瘤所致便秘患者,采用随机数字表法分为对照组和观察组,各38例。对照组患者采用乳果糖口服液治疗,观察组患者采用自拟通便散外敷神阙治疗。比较两组患者的临床疗效、便秘症状评分、不良事件发生情况及治疗前后生活质量评分。结果观察组治疗总有效率94.74%高于对照组的78.95%,差异具有统计学意义(P<0.05)。观察组粪便性状,排便时间,排便费力,排便频率,腹胀及下坠、不尽、胀感评分与总分均明显低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组躯体功能、社会功能、心理功能及角色功能评分均高于治疗前,且观察组躯体功能评分(73.25±6.31)分、社会功能评分(74.55±6.62)分、心理功能评分(75.65±7.54)分及角色功能评分(76.28±7.15)分均显著高于对照组的(65.86±6.62)、(66.74±6.45)、(64.75±7.12)、(67.72±6.88)分,差异具有统计学意义(P<0.05)。两组不良事件发生率比较差异无统计学意义(P>0.05)。结论对晚期肿瘤所致便秘患者进行自拟通便散外敷神阙治疗具有积极疗效,可有效缓解临床症状,改善患者生活质量,且不会增加不良事件,值得临床推广。Objective To study the clinical effect of external application of self-made Tongbian San on Shenque acupoint in the treatment of constipation caused by advanced tumors.Methods A total of 76 patients with constipation caused by advanced tumor were divided into control group and observation group by random numerical table,with 38 cases in each group.Patients in the control group were treated with lactulose oral solution,and patients in the observation group were treated with external application of self-made Tongbian San on Shenque acupoint.The clinical efficacy,constipation symptom scores,occurrence of adverse events and quality of life scores before and after treatment were compared between the two groups.Results The total effective rate of treatment 94.74%in the observation group was higher than 78.95%in the control group,and the difference was statistically significant(P<0.05).The fecal properties,defecation time,defecation effort,defecation frequency,abdominal distension and falling,unfinished feeling,feeling of distention score and total score in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the physical function,social function,psychological function and role function scores of the two groups were higher than those of this group before treatment;the physical function score of the observation group was(73.25±6.31)points,the social function score was(74.55±6.62)points,the psychological function score was(75.65±7.54)points and the role function score was(76.28±7.15)points,which were significantly higher than(65.86±6.62),(66.74±6.45),(64.75±7.12),and(67.72±6.88)points of the control group;all the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion External application of self-made Tongbian San on Shenque acupoint has a positive effect on patients with constipation caused by ad
