家兔急性右心衰竭模型建立的实验研究

摘　　要：目的探索家兔急性右心衰竭模型复制中氢氧化铁注入浓度及剂量的最佳条件,明确它们在右心衰竭形成过程中的作用,优化家兔急性右心衰竭模型。方法对照组家兔未注射氢氧化铁;氢氧化铁设计2个不同浓度,3个不同剂量,得到6个实验组,实验组家兔麻醉、插管、注入氢氧化铁后,观察家兔口唇皮肤颜色、呼吸频率(RF)、心率(HR)、血压(BP)、中心静脉压(CVP)、右室收缩压(RVSP)、右心功能指标[+dp/dtmax(右室压力上升最大速率)、右室舒张压(RVDP)、-dp/dtmax(右室压力下降最大速率)]等栓塞前后变化情况,紧接着快速输入生理盐水后,观察家兔口唇皮肤颜色、RF、HR、BP、CVP、右心功能指标等变化情况,复制家兔急性右心衰竭模型。结果各实验组家兔均发生不同程度的急性右心衰竭,栓塞成功,与对照组比较,各实验组家兔口唇紫绀、皮肤黏膜苍白;CVP、RVDP、-dp/dtmax显著增加(P<0.05),BP、RVSP、+dp/dtmax显著降低(P<0.05);分别与实验组5和实验组6相比,对照组、实验组1、实验组2、实验组3家兔RVSP、+dp/dtmax显著增加(P<0.05)。栓塞成功并注入10 mL、30 mL、50 mL、70 mL生理盐水后,分别与实验组5和实验组6相比,对照组、实验组1、实验组2、实验组3家兔CVP显著下降(P<0.05);此外,栓塞成功并注入90 mL生理盐水后,分别与实验组5和实验组6相比,对照组、实验组1、实验组2、实验组3家兔CVP、RVDP、-dp/dtmax均显著下降(P<0.05);与其他组家兔相比,实验组5和实验组6家兔胸水、腹水、肝腹水程度更严重,心腔体积更大。结论用氢氧化铁进行肺栓塞能成功复制家兔急性右心衰竭模型,掌握氢氧化铁浓度、剂量的最佳条件是成功复制家兔急性右心衰竭模型的前提,以1 mL/kg氢氧化铁(4%)进行家兔肺栓塞辅助输液后HR和BP相对稳定,复制家兔急性右心衰竭模型效果最佳。Objective To explore the optimal conditions of the concentration and dose of ferric hydroxide in the replication of rabbit model of acute right heart failure,clarify their roles in the formation process of right heart failure,and optimize the model of rabbit acute right heart failure.Methods Rabbits in the control group were not injected with ferric hydroxide;two different concentrations and three different doses of ferric hydroxide were designed,to obtain six experimental groups.After the rabbits of the experimental groups were anesthetized,intubated and injected with ferric hydroxide,the changes of rabbit lip skin color,respiratory rate(RF),heart rate(HR),blood pressure(BP),central venous pressure(CVP),and right ventricular indexes[right ventricular systolic pressure(RVSP),maximum rate of right ventricular pressure rise(+dp/dtmax),right ventricular diastolic pressure(RVDP),maximum rate of right ventricular pressure decline(-dp/dtmax)]before and after embolization were observed.Immediately after the rapid administration of normal saline,the rabbit lip skin color,RF,HR,BP CVP,and right ventricular indexes were observed,and the rabbit model of acute right heart failure was replicated.ResultsAll rabbits in the experimental groups had different degrees of acute right heart failure.After successful embolization,compared with the control group,rabbits in each experimental group had cyanosis on lips and pale skin mucosa;the CVP,RVDP and-dp/dtmax increased significantly(P<0.05),while the BP,RVSP and+dp/dtmax decreased significantly(P<0.05);compared with the experimental group 5 and the experimental group 6,the RVSP and+dp/dtmax of rabbits in the control group,the experimental group 1,the experimental group 2 and the experimental group 3 increased significantly(P<0.05).After successful embolization and injection of 10 mL,30 mL,50 mL and 70 mL of normal saline,compared with the experimental group 5 and the experimental group 6,the CVP of rabbits in the control group,the experimental group 1,the experimental group 2 and th
