不同疗法在子宫内膜息肉术后的应用效果比较

摘　　要：目的:比较屈螺酮炔雌醇片(Ⅱ)、去氧孕烯炔雌醇片、左炔诺孕酮宫内节育系统在子宫内膜息肉(EP)术后的应用效果及其对Ki-67表达及对激素水平的影响。方法:选取2019年3月至2020年10月期间于洛阳市妇幼保健院接受治疗的150例EP患者,按照术后接受的治疗方案不同分成A组〔屈螺酮炔雌醇片(Ⅱ)〕、B组(去氧孕烯炔雌醇片)、C组(左炔诺孕酮宫内节育系统),各50例。比较三组患者的术后治疗效果、Ki-67表达及性激素水平。结果:C组患者术后6个月、12个月的复发率明显低于A组、B组,差异具有统计学意义(P<0.05);C组术后3个月、术后6个月、术后12个月的子宫内膜厚度均薄于A组、B组,差异具有统计学意义(P<0.05);C组术后6个月子宫内膜Ki-67的3级表达者占比少于A组、B组,雌激素受体(ER)、孕激素受体(PR)3级患者占比明显高于A组、B组,差异均具有统计学意义(P<0.05)。结论:左炔诺孕酮宫内节育系统在EP术后的应用效果及对Ki-67表达、激素水平的影响,均明显优于屈螺酮炔雌醇片(Ⅱ)、去氧孕烯炔雌醇片。Objective To compare the effects of drospirone ethinylestradiol tablet(Ⅱ),deoxygestrel ethinylestradiol tablet and levonorgestrel intrauterine contraceptive system on Ki-67 expression and hormone level after endometrial polyp(EP).Methods A total of 150 patients with EP who received treatment in Luoyang Maternal and Child Health Hospital from March 2019 to October 2020 were selected and divided into group A[drospirenone ethoestradiol tablet(Ⅱ)],group B(deoxygestrel ethoestradiol tablet)and group C(levonorgestrel intrauterine contraception system)according to different postoperative treatment plans,with 50 patients in each group.The postoperative treatment effect,Ki-67 expression and sex hormone levels were compared among the three groups.Results The recurrence rate of group C at 6 and 12 months after surgery was significantly lower than that of the group A and group B,the difference was statistically significant(P<0.05).The endometrial thickness of group C was thinner than that of the group A and group B at 3 months,6 months and 12 months after surgery,with statistical significance(P<0.05).6 months after operation,the proportion of grade 3 expression of endometrium Ki-67 in group C was lower than that in the group A and group B,and the proportion of estrogen receptor(ER)and progesterone receptor(PR)grade 3 patients was significantly higher than that in group A and group B,with statistical significance(P<0.05).Conclusion The application effect of levonorgestrel intrauterine contraceptive system after EP and the effect on Ki-67 expression and hormone level were obviously better than the drospirone ethinylestradiol tablets(Ⅱ)and deoxygestrel ethinylestradiol tablets.
