射频消融和冷冻消融对心房颤动患者生活质量和心理状况的影响

摘　　要：目的探讨射频消融与冷冻消融对心房颤动患者生活质量、心理状况的影响。方法选取2018年1月至2021年3月至郑州大学第一附属医院心血管内科就诊,于治疗期间接受消融手术的100例心房颤动患者,采用随机数字表法将患者分为射频消融组和冷冻消融组,每组50例。两组分别按照标准手术流程完成消融手术,比较两组术前及术后3个月生活质量调查表(SF-36)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分变化情况。结果冷冻消融组总有效率为92.0%,射频消融组总有效率为84.0%,差异无统计学意义(P>0.05)。相较于治疗前,两组术后3个月SF-36、HAMA、HAMD评分均有改善(P<0.05),且冷冻消融组改善优于射频消融组(P<0.05)。结论冷冻消融与射频消融治疗心房颤动有效性相当。两种消融方法均可改善患者生活质量及心理状态。相较于射频消融,冷冻消融改善心房颤动患者生活质量和心理状况效果更加明显。Objective To investigate the effects of radiofrequency ablation and cryoablation on quality of life and psychological status of patients with atrial fibrillation.Methods From January 2018 to March 2021,100 patients who underwent ablation in the First Affiliated Hospital of Zhengzhou University were randomly divided into cryoablation group and radiofrequency ablation group,50 cases in each group.The two groups completed ablation according to the standard procedure.The levels of 36-itemshort-form health survey(SF-36),Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)were compared before and 3 months after ablation.Results The total effective rate was 92.0% in the cryoablation group and 84.0% in the radiofrequency ablation group,and there was no difference between the two groups(P>0.05).Compared with baseline,SF-36,HAMA and HAMD of the two groups were improved 3 months after operation(P<0.05),and the cryoablation group was better than the radiofrequency ablation group(P<0.05).Conclusion Cryoablation and radiofrequency ablation have similar total effective rate in atrial fibrillation treatment.Both ablation methods can improve the quality of life and mental state.Compared with radiofrequency ablation,cryoablation has a higher effective rate in improving quality of life and psychological status.
