乙型肝炎肝硬化患者内皮祖细胞数量、功能变化及与Child-Pugh评分的相关性

摘　　要：目的探讨乙型肝炎肝硬化患者循环内皮祖细胞(EPC)的数量、功能变化及与Child-Pugh评分的相关性。方法纳入乙型肝炎肝硬化患者60例,其中Child-Pugh A、B、C级各20例,另选20例健康体检者为对照组。计算患者Child-Pugh评分,采外周血分离提取EPC,检测EPC在体外的迁移、增殖能力。结果乙型肝炎肝硬化早期患者(Child-Pugh A、B级)循环EPC数量较对照组增加(P<0.05),Child-Pugh C级患者循环EPC数量较对照组高但差异无显著性(P>0.05)。乙型肝炎肝硬化患者循环EPC迁移、增殖能力均较对照组下降(P<0.05),且与Child-Pugh评分呈负相关(P<0.05)。结论乙型肝炎肝硬化患者EPC的动员能力增强,但EPC迁移、增殖能力减弱,可能与乙型肝炎肝硬化肝损伤程度相关。Aim To investigate the change of the number and function of circualting endothelial progenitor cell(EPC)and its correlation with Child-Pugh scorein patients with hepatitis B cirrhosis.Methods Sixty patients with hepatitis B cirrhosis were enrolled,including 20 Child-Pugh A patients,20 Child-Pugh B patients,and 20 Child-Pugh C grades.Another 20 healthy volunteers as the control group.Calculate Child-Pugh score,detect the migration and proliferation of EPC in vitro.Results The number of circulating EPC in patients with early hepatitis B cirrhosis(Child-Pugh A,B)was significantly higher than that of the healthy control group(P<0.05),and the number of circulating EPC in Child-Pugh C level patients was higher than that of the healthy control group,but there was no difference between the two groups(P>0.05).The migration and proliferation of circulating EPC in patients with hepatitis B cirrhosis were significantly lower than those in healthy control group(P<0.05),and they were negatively correlated with Child-Pugh score(P<0.05).Conclusion The mobilization ability of EPC in patients with hepatitis B cirrhosis is enhanced.But the migration and proliferation of EPC is weakened,which may be related to the severity of hepatitis B cirrhosis.
