摘 要:目的探讨胎儿矢状切面、冠状面鼻后三角(RNT)、上颌骨轴平面在孕11~13^(+6)周(妊娠早期)腭裂筛查中的应用价值。方法对16例妊娠早期诊断腭裂患儿以及7例妊娠早期漏诊患儿进行回顾性分析,分析胎儿矢状切面、RNT、上颌骨轴平面声像图有无腭裂征象。结果16例妊娠早期诊断腭裂患儿中包括正中腭裂4例、单侧腭裂8例及双侧腭裂4例,矢状切面、RNT、上颌骨轴平面的获取率分别约100%(16/16)、93.7%(15/16)、75.0%(12/16),各个切面发现腭裂征象的阳性率分别为87.5%(14/16)、93.3%(14/15)、100%(12/12)。7例妊娠早期漏诊患儿,包括单侧腭裂5例、双侧腭裂2例,矢状切面、RNT、上颌骨轴平面的获取率分别约100%(7/7)、28.6%(2/7)、0%(0/7),矢状切面、RNT阳性率分别约42.9%(3/7)、0%(0/2)。结论矢状切面、冠状面鼻后三角切面、上颌骨轴平面在妊娠早期腭裂筛查中均具有重要价值,联合应用可提高诊断腭裂的准确性。Objective To explore the application value of fetal sagittal section,coronal posterior nasal triangle(RNT)and maxillary axial plane in cleft palate screening during 11~13^(+6) weeks of pregnancy(early pregnancy).Methods 16 cases of cleft palate diagnosed in early pregnancy and 7 cases of missed diagnosis in early pregnancy were retrospectively analyzed.The signs of cleft palate in fetal sagittal section,RNT and plane sonogram of maxillary axis were analyzed.Results 16 cases of cleft palate were diagnosed in early pregnancy,including 4 cases of median cleft palate,8 cases of unilateral cleft palate and 4 cases of bilateral cleft palate.The acquisition rates of sagittal plane,RNT and maxillary axial plane were about 100%(16/16),93.7%(15/16)and 75.0%(12/16),respectively.The positive rates of cleft palate signs in each plane were 87.5%(14/16),93.3%(14/15),and 100%(12/12),respectively.There were 7 cases of missed diagnosis in early pregnancy,including 5 cases of unilateral cleft palate and 2 cases of bilateral cleft palate.The acquisition rates of sagittal plane,RNT and axial plane of maxilla were about 100%(7/7),28.6%(2/7)and 0%(0/7),respectively,and the positive rates of sagittal plane and RNT were about 42.9%(3/7)and 0%(0/2),respectively.Conclusion Sagittal plane,RNTand axial plane of maxilla all play an important role in the screening of cleft palate in early pregnancy,and combined application can improve the accuracy of cleft palate diagnosis.
