海口市2008-2017年男男性行为哨点监测HIV感染率变化趋势及原因分析

摘　　要：目的掌握2008-2017年海口市MSM人群HIV感染状况,探讨该人群HIV感染变化趋势及分析原因,为制定科学的防治措施提供依据。方法于2008-2017年期间的每年4-7月,在固定的自愿咨询检测(VCT)门诊连续对前来咨询检测的MSM人群收集相关信息,采集3~5 mL静脉血,采用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)进行HIV抗体检测,运用描述性分析MSM社会人口学特征,采用χ^(2)检验进行HIV抗体阳性率的比较和线性趋势检验HIV抗体阳性率的变化。结果2008年4月-2017年7月共调查MSM 3214例,年龄13~70岁,年龄中位数25岁,检出HIV抗体阳性160例,HIV抗体阳性率为4.98%,2008-2017年各年度HIV抗体阳性率为1.75%~7.98%,最高年度是2013年。10年间HIV抗体阳性率增幅为141.65%,年均增长速度14.16%。2008-2013年MSM人群HIV抗体阳性率呈上升趋势(χ^(2)趋势=16.670,P<0.01),≤24岁年龄组和25~39岁组、未婚或离异丧偶、初中及以下和大专及以上文化、本省户籍、本地居住时间≥2年组阳性率呈上升趋势(P<0.05)。HIV检出率上升期,未婚或离异丧偶组阳性率增幅250.54%,≤24岁年龄组阳性率增幅283.57%,大专及以上组阳性率增幅353.33%,本省户籍组阳性率增幅208.33%,本地居住时间≥2年组阳性率增幅478.59%。2015-2017年MSM人群HIV抗体阳性率差异有统计学意义(χ^(2)=7.542,P<0.05)。结论海口市MSM人群哨点HIV感染率2008-2013年呈上升趋势,2014-2017年趋于平缓,是开展健康教育和干预的关键时期,应继续强化针对性健康教育和干预力度,遏制艾滋病快速蔓延。Objective To summarize the infection status of MSM(men who had sex with men)population in Haikou from 2008 to 2017,and anaylze the changing trend and the causes of HIV infection,so as to provide reference for the development of scientific prevention and control measures.Methods The period during April to July in every year from 2008 to 2017,cross-sectional survey were conducted to collect relevant information of MSM in the regular voluntary counseling and testing(VCT)clinic.Venous blood(3-5 mL)was collected,and HIV antibody detection was made by enzyme-linked immunosorbent assay(ELISA).The social and demographic characteristics of MSM were analyzed descriptively,and the positive rate of HIV antibody was compared by χ^(2) test and the change of HIV antibody positive rate was tested by linear trend.Results From April 2008 to July 2017,3214 MSM people were investigated,the youngest was 13 years old,the oldest was 70 years old,and the median age was 25 years old.In total 160 cases of HIV antibody positive were detected,and the positive rate of HIV antibody was 4.98%.The annual positive rate of HIV antibody was 1.75%-7.98% from 2008 to 2017,with the highest in 2013.The positive rate of HIV antibody increased by 141.65% in 10 years,with an average annual growth rate of 14.16%.The positive rate of HIV antibody in MSM population increased from 2008 to 2013(χ^(2)trend=16.670,P<0.01).The positive rates of≤24 years old group,25-39 years old group,unmarried or divorced widowed group,junior high school or below group,college education group,local household registration group,local residence≥2 years group were increased.During the increasing period,the positive rate of unmarried or divorced widowed group increased by 250.54%,the positive rate of ≤24 years old group increased by 283.57%,the positive rate of college and upper group increased by 353.33%,the positive rate of household registration group increased by 208.33%,the positive rate of living time ≥2 years group increased by 478.59%.There was significant differe
