摘　　要：患者男,68岁,全身暗紫色斑块1年,无自觉症状。系统检查无异常。皮肤科情况:全身散在分布数枚直径0.2~3.7 cm的暗紫色斑块,部分斑块表面附着有鳞屑。皮损组织病理示:表皮层水肿,真皮全层可见组织细胞增生,伴淋巴细胞浸润。免疫组织化学检查示:CD20、CD3、CD68(KP1)、CD68(PGM1)阳性,S-100、CD117、FactorⅫⅠa阴性。诊断:非朗格汉斯组织细胞增生症。口服沙利度胺和醋酸泼尼松后,皮损好转。A 68-year-old male presented with asymptomatic dark purple patches on the whole body for one year.Systematic physical examination showed no abnormality.Dermatological examination showed several dark purple plaques 0.2-3.7 cm in size with adhesive scales on his whole body.Histopathology revealed epidermis edema and that the whole layer of the dermis was infiltrated by histiocytes and a few lymphocytes.Immunohistochemical staining was positive for CD20,CD3 and CD68 but negative for S-100,CD117 and factor ⅫⅠa.Diagnosis: non-Langerhans cell histiocytoses.After treatment with thalidomide and prednisone acetate, the skin lesions improved.
