摘 要:目的探讨体重指数(BMI)对行主动脉瓣置换术(SAVR)的单纯主动脉瓣狭窄(AS)患者早期预后的影响。方法回顾性分析安徽医科大学第二附属医院心脏大血管外科2019年6月至2020年6月间51例行SAVR的患者的临床资料,根据其BMI分为3组:体重正常组(18.5 kg/m^(2)<BMI≤23.9 kg/m^(2)),超重组(24.0 kg/m^(2)<BMI<28.0 kg/m^(2)),肥胖组(BMI≥28.0 kg/m^(2))。分析3组患者各项指标的差异以及BMI对于SAVR术后早期预后的影响。结果肥胖组患者术前的糖尿病患病率、高血压患病率、血红蛋白浓度、左室内径(LV),左房内径(LA),右室内径(RV)均高于体重正常组,差异有统计学意义(P<0.05)。多重线性回归分析提示:患者的BMI与重症监护时间(r^(2)=0.554,P=0.015)及住院时间(r^(2)=0.490,P=0.014)呈正相关性,肥胖组的重症监护时间(P=0.009)及住院时间(P=0.010)均较体重正常组明显增加。结论AS患者术前BMI水平与SAVR术后在重症医学科滞留时间和住院时间有关。Objective To explore the influences of BMI on the early prognosis of patients with simple AS who required SAVR.Methods Fifty-one patients who underwent SAVR in the Department of Cardiovascular Surgery,the Second Hospital of Anhui Medical University from June 2019 to June 2020,were analyzed retrospectively.The patients were divided into three groups according to their BMI pre-operatively:normal group(18.5 kg/m^(2)<BMI≤23.9 kg/m^(2)),overweight group(24.0 kg/m^(2)<BMI<28.0 kg/m^(2)),obesity group(BMI≥28.0 kg/m^(2)).The clinical data was collected and analyzed to compare the differences in various indicators of the three groups of patients and explore the impact of BMI on the early prognosis after SAVR.Results The prevalence of diabetes,prevalence of hypertension,preoperative hemoglobin concentration,LV,LA,and RV of the obese group were higher than those of the normal weight group,and the difference was statistically significant.Multiple linear regression analysis showed that there was a positive correlation between the patient’s BMI and the duration of intensive care time(r^(2)=0.554,P=0.015)and the length of stay(r^(2)=0.490,P=0.014).Compared with the normal weight group,the intensive care time(P=0.009)and length of stay(P=0.010)of the obese group were increased significantly.Conclusion Applicable BMI could shorten the duration of intensive care time and the length of stay in AS patients after SAVR.
