甘肃省农村医疗卫生资源配置的公平性研究

摘　　要：目的 分析甘肃农村卫生资源配置公平性,为甘肃农村医疗卫生资源配置规划制定提供依据。方法 通过2015—2019年甘肃农村卫生资源年增长情况分析甘肃卫生资源整体状况,采用基尼系数、集聚度衡量医疗卫生资源配置的公平性。结果 2015—2019年,甘肃农村医疗卫生机构数有所减少,床位数和卫生人员数量显著增加。按人口配置的卫生机构卫生、床位的基尼系数均小于0.5000,卫生人员的基尼系数在0.4321~0.5002;按地理配置的基尼系数均大于0.7000。张掖、平凉地区农村各类卫生资源地理与人口集聚度差值均大于0;庆阳等地卫生机构与卫生人员地理与人口集聚度的均差值小于0。结论 甘肃省农村医疗卫生资源按人口配置的公平性优于按地理配置的公平性,不同市域间卫生资源配置公平性差异显著。Objective:To analyze the equity of rural health resource allocation,and provide the basis for the development of rural health resource allocation planning in Gansu Province.Method:The overall situation of health resources was analyzed,and the Gini coefficient and agglomeration degree were used to measure the equity of medical and health resource allocation through the annual growth of rural health resources in Gansu Province from 2015 to 2019.Result:From 2015 to 2019,the number of rural medical and health institutions in Gansu decreased,while the numbers of beds and health workers increased significantly.The Gini coefficient of health institutions and beds allocated according to population was less than 0.5000,and the Gini coefficient of health workers ranged from 0.4321 to 0.5002.The Gini coefficient of geographic configuration is greater than 0.7000.In Zhangye and Pingliang areas,the difference between the geography of all kinds of health resources in rural areas and population agglomeration degree is greater than 0.And the mean difference between the geography of health institutions and health workers,and population agglomeration degree in Qingyang and the other areas was less than 0.Conclusion:In Gansu Province,the equity of rural medical and health resource allocation according to population is better than that according to the geography,and there are significant differences among different cities in the equity of health resource allocation.
