摘 要:为解决患者预约专家号难、看病难、入住病房难的困境,医院结合自身实际情况融入全病程服务管理模式,创新知名专家团队模式,畅通门诊住院诊疗一体化流程,实现了医院内部资源与流程的优化以及对患者的延续性诊疗,是优化院内层级转诊,落实院外医联体转诊的有益实践与探索,真正提高了患者的就医体验,同时也为医疗机构整合医疗资源、推动分级诊疗的实施提供了思路。In order to solve the difficulties of making an appointment with experts,seeing a doctor and being admitted into hospital,the hospital,combining with its own actual situation,integrates the whole-course service management mode,innovates the well-known expert team mode,and keeps the process of the integrated outpatient and inpatient treatment unobstructed,which has realized the optimization of internal resources and processes of the hospital,and the continuity of diagnosis and treatment for patients.It is a beneficial practice and exploration to optimize in-hospital referral and implement out-hospital referral within hospital union.It really improves patients’medical experience,and also provides ideas for medical institutions to integrate medical resources and promote the implementation of the graded diagnosis and treatment.
