疏肝降脂片改善乳腺癌内分泌治疗期血脂异常临床观察

摘　　要：目的:观察疏肝降脂片对芳香化酶抑制剂治疗期乳腺癌患者血脂和肝郁证候的影响。方法:回顾性纳入符合标准的乳腺癌患者79例,分为观察组39例和对照组40例,观察组服用疏肝降脂片配合饮食疗法,对照组采取饮食运动疗法,两组疗程均为6周,观察比较治疗前后两组患者中医证候积分和广泛性焦虑量表7(GAD-7量表)评分,检测两组患者治疗前后血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白(Apo)A1、ApoB、脂蛋白(a)[LP(a)]水平,比较两组降脂有效率,评估疗效,观察不良反应情况。结果:治疗后两组中医证候积分均降低(P<0.05),且观察组低于对照组(P<0.05);治疗后对照组各血脂指标无明显改善(P>0.05),观察组TC、TG、LDL-C、非HDL/HDL、ApoB水平降低(P<0.05),且TC、LDL-C、ApoB水平低于对照组(P<0.05);治疗后两组焦虑评分均降低(P<0.05),但组间比较差异无统计学意义(P>0.05);观察组治疗总有效率79.49%,对照组40.00%,两组疗效比较差异有统计学意义(P<0.05);两组不良反应发生情况相当。结论:疏肝降脂片可降低内分泌治疗期乳腺癌患者血脂水平,缓解症状,改善焦虑情况,安全性好。Objective:To observe the effect of Shugan Jiangzhi Pill(疏肝降脂片)on breast cancer patients with dyslipidemia and liver depression syndrome during aromatase inhibitor(AI)treatment.Methods:A total of 79 patients with breast cancer meeting the criteria were included retrospectively,and were divided into the observation group(39 patients)and control group(40 patients).The observation group was given Shugan Jiangzhi Pill and dietotherapy,while the control group was given dietotherapy and exercise therapy.Both groups were treated for 6 weeks.Before and after treatment,traditional Chinese medicine(TCM)syndrome score,and generalized anxiety disorde-7(GAD-7)score were compared between two groups,and levels of serum total cholesterol(TC),triglyceride(TG),low density lipoprotein-cholesterol(LDL-C),high density lipoprotein-cholesterol(HDL-C),apolipoprotein(Apo)A1,Apo B,and lipoprotein a[Lp(a)]were examined to compare the effect of reducing blood lipid,so as to evaluate the effect.The adverse reactions of two groups were observed.Results:After the treatment,TCM syndrome scores in both groups were reduced(P<0.05),and those scores were lower in the observation group than those in the control group(P<0.05).In the control group,indexes of the blood lipid showed no change(P>0.05),while levels of serum TC,TG,LDL-C,non-HDL/HDL,ApoB in the observation group were reduced(P<0.05),and serum levels of TC,LDL-C and Apo B in the observation group were lower than those in the control group.CAD-7 scores were reduced in both groups,but without statistically significant difference between both groups(P>0.05).The total efficacy in the observation was 79.49%,and in the control group was 40.00%,with statistically significant difference between both groups.Treatments in both groups showed the same safety.Conclusion:Shugan Jiangzhi Pill could reduce the levels of blood lipids,improve symptoms and relieve anxiety in breast cancer patients with endocrine therapy with high safety.
