双嘧达莫联合人免疫球蛋白及阿司匹林治疗川崎病临床观察

摘　　要：目的探讨双嘧达莫联合人免疫球蛋白及阿司匹林治疗川崎病的临床疗效,以及对患儿血清降钙素原(PCT)、白细胞介素6(IL-6)及T淋巴细胞亚群水平的影响。方法选取海南省琼中黎族苗族自治县人民医院2017年5月至2019年12月收治的川崎病患儿120例,按随机数字表法分为观察组和对照组,各60例。两组患儿均予阿司匹林肠溶片和冻干静注人免疫球蛋白(pH 4),观察组患儿加用双嘧达莫片,均治疗8周。结果观察组总有效率为91.67%,显著高于对照组的78.33%(P<0.05)。治疗后,观察组患儿血清IL-6、PCT、肿瘤坏死因子-α、C反应蛋白水平均显著低于对照组(P<0.05);血小板计数、白细胞计数、红细胞沉降率均显著低于对照组(P<0.05);外周血T淋巴细胞亚群CD3+,CD4+水平和CD4+/CD8+均显著高于对照组(P<0.05),CD8+水平显著低于对照组(P<0.05)。观察组和对照组治疗期间药品不良反应发生率相当(8.33%比13.33%,P>0.05)。结论双嘧达莫联合人免疫球蛋白及阿司匹林治疗川崎病疗效良好,可减轻患儿的炎性反应,改善凝血功能,提高免疫功能,且安全性较高。Objective To investigate the clinical efficacy of dipyridamole combined with human immunoglobulin and aspirin in the treatment of Kawasaki disease(KD),and the effect on levels of proclacitonin(PCT),interleukin-6(IL-6)and T lymphocyte subsets in children with KD.Methods A total of 120 children with KD admitted to the People′s Hospital of Qiongzhong Li and Miao Autonomous County from May 2017 to December 2019 were selected and divided into the observation group and the control group according to the random number table method,with 60 cases in each group.The children in the two groups were given Aspirin Enteric-coated Tablets and Human Immunoglobulin(pH 4)for Intravenous Injection,Freeze-dried,on this basis,the children in the observation group were given Dipyridamole Tablets.Both groups were treated for eight weeks.Results The total effective rate in the observation group was 91.67%,which was significantly higher than 78.33%in the control group(P<0.05).After treatment,the levels of serum IL-6,PCT,tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)were significantly lower than those in the control group(P<0.05).The platelet count(PLT),white blood cell count(WBC)and erythrocyte sedimentation rate(ESR)in the observation group were significantly lower than those in the control group(P<0.05).The levels of peripheral blood T lymphocyte subsets CD3+,CD4+and CD4+/CD8+in the observation group were significantly higher than those in the control group(P<0.05),while the level of CD8+in the observation group was significantly lower than that in the control group(P<0.05).The incidence of adverse drug reactions in the observation group was similar to that in the control group(8.33%vs.13.33%,P>0.05).Conclusion Dipyridamole combined with human immunoglobulin and aspirin is effective and safe in the treatment of KD,which can reduce the inflammatory reaction,improve the coagulation function and immune function of children.
