摘 要:目的:探究干细胞白血病基因(Stem Cell Leukemia,SCL)重组慢病毒和缝隙连接蛋白基因[Gap Junction Protein Alpha 1,GJA1(又名Connexin43,Cx43)]重组慢病毒联合作用对豚鼠糖尿病膀胱(Diabetic Cystipathy,DCP)逼尿肌的影响。方法:40只健康豚鼠适应性喂养1周后,按照200mg/kg一次性腹腔注射链脲佐菌素(Streptozotocin,STZ),定期每周监测随机血糖,正常饲养12周后,通过尿流动力学检查筛选出40只符合标准的豚鼠,将其随机分成4组:糖尿病组、SCL慢病毒组、Cx43慢病毒组、SCL+Cx43慢病毒组。糖尿病组经尿道向膀胱内灌注0.2mL不含基因的空慢病毒,SCL组、Cx43组使用同样方法分别灌注SCL慢病毒和Cx43慢病毒0.2mL,SCL+Cx43慢病毒组经尿道灌注SCL、Cx43慢病毒各0.2mL。转染14天后进行尿流动力学检查,检查后处死豚鼠,取出豚鼠膀胱,制作逼尿肌肌条,行逼尿肌肌条收缩实验。结果:SCL组和Cx43组与糖尿病组相比,在尿流动力学检查中逼尿肌压力无明显差异(P>0.05),SCL+Cx43组较糖尿病组尿流动力学检查可见逼尿肌压力有明显改善(P<0.05)。在逼尿肌肌条实验中,糖尿病组较正常组逼尿肌收缩幅度及频率明显下降(P<0.05),SCL组与糖尿病组相比逼尿肌收缩幅度及频率有所改善(P<0.05),Cx43组与糖尿病组相比,收缩幅度及收缩频率未见改善(P<0.05),SCL组和Cx43组之间比较无统计学差异(P>0.05)。SCL+Cx43组与糖尿病组相比,逼尿肌收缩幅度及频率有明显差异(P<0.05),SCL+Cx43组与正常组相比差异有统计学意义(P<0.05)。结论:经尿道联合灌注SCL+Cx43基因重组慢病毒可以成功在豚鼠DCP膀胱中转染成功,可提高糖尿病膀胱逼尿肌压力,离体试验证实联合灌注较单一灌注可提高逼尿肌收缩幅度及收缩频率,为临床治疗DCP提供新的思路。Objective:To investigate the effects of the combination of Stem Cell Leukemia(SCL)recombinant lentivirus and Gap Junction Protein Alpha1(GJA1(aka Connexin43,Cx43))recombinant lentivirus on the detrusor muscle of the guinea pig diabetic cystipathy(DCP).Methods:Forty healthy guinea pigs were fed normally for 1 week,and Streptozotocin(STZ)was injected intraperitoneally once according to the standard of 200 mg/kg.After injection,random blood glucose was monitored regularly on a weekly basis,and after 12 weeks of normal feeding,40 eligible guinea pigs were selected by urodynamic examination and randomly divided into 4 groups:the diabetic group,the SCL lentiviral group,the Cx43 lentiviral group,and the SCL+Cx43 lentiviral group.The diabetic group instilled 0.2 mL of empty lentivirus without gene into the bladder through the urethra,the SCL and Cx43 groups instilled 0.2 mL of SCL lentivirus and Cx43 lentivirus,respectively,in the same way,and the SCL+Cx43 lentivirus group instilled 0.2 mL each of SCL and Cx43 lentivirus through the urethra.Urodynamic examination was performed 14 days after perfusion,and the guinea pig was executed after the examination and their bladders were removed.The removed bladders were made into bladder smooth muscle strips,and bladder smooth muscle strips contraction experiments were performed.Results:In urodynamic examination,there was no significant difference in detrusor pressure in the SCL and Cx43 groups compared with the diabetic group(P>0.05),and a significant improvement in detrusor pressure was seen in the SCL+Cx43 group compared with the diabetic group(P<0.05).In the detrusor strip experiment,the amplitude and frequency of detrusor strip contraction decreased significantly in the diabetic group compared with the normal group(P<0.05).The amplitude and frequency of detrusor strip contraction improved in the SCL group compared with the diabetic group(P<0.05).The amplitude and frequency of contraction did not improved in the Cx43 group compared with the diabetic group(P>0.05),and there was
