妊娠早中期血清Hcy、FA、VitB12与子痫前期的相关性分析

摘　　要：目的:分析妊娠早中期孕妇血清同型半胱氨酸(Hcy)、叶酸(FA)、维生素B12(VitB12)与子痫前期(PE)的相关性。方法:选取2020年09月-2021年08月于本院行产前检查的孕30周以内孕妇237例为研究对象,检测血清Hcy、FA、VitB12,根据妊娠期血压及症状、尿蛋白水平分为PE组35例、重度PE组29例和正常组173例,比较正常组、PE组与重度PE组妊娠早中期上述各指标水平,分析不同孕周孕妇血清Hcy、FA、VitB12与PE严重程度的相关性。结果:重度PE组在妊娠早中期其血清FA、VitB12水平显著低于PE组、正常组,血清Hcy水平显著高于PE组、正常组(P<0.05);PE组血清FA、VitB12水平显著低于正常组,血清Hcy水平显著高于正常组(P<0.05)。Pearson线性相关分析结果显示妊娠早中期血清Hcy与病情严重程度呈正相关关系(P<0.05);妊娠早中期VitB 12、FA与病情严重程度呈负相关关系(P<0.05)。结论:PE在尚未出现临床症状及相关并发症时,妊娠早中期血清Hcy、FA、VitB12水平已出现紊乱,血清Hcy、FA、VitB12与PE的病情进展有一定相关性。Objective:To analyze the correlation between serum homocysteine(Hcy),folic acid(FA),vitamin B12(VitB12)and preeclampsia(PE).Methods:A total of 273 pregnant women within 30 weeks of pregnancy who underwent antenatal examination in our hospital from September 2020 to August 2021 were selected as the research objects.The serum Hcy,FA,VitB12 were measured.According to the blood pressure,symptoms and urinary protein level during pregnancy,they were divided into severe PE group(n=29),PE group(n=35)and normal group(n=173).The levels of the above indexes in normal group,mild PE group and severe PE group in the early and second trimester of pregnancy were compared,the correlation between serum Hcy,FA,VitB12 and PE severity in pregnant women at different gestational weeks was analyzed.Results:The levels of serum FA and VitB12 in severe PE group were significantly lower than those in PE group and normal group in early and middle pregnancy,and the level of serum Hcy in severe PE group was significantly higher than those in PE group and normal group(P<0.05);The levels of serum FA and VitB12 in PE group were significantly lower than those in normal group in early and middle pregnancy,and the level of serum Hcy was significantly higher than those in normal group(P<0.05).Pearson linear correlation analysis showed that Hcy in early and middle pregnancy was positively correlated with the severity of PE(P<0.05);There was a negative correlation between VitB 12 and FA and the severity of PE in early and middle pregnancy(P<0.05).Conclusion:When there are no clinical symptoms and related complications of PE,the serum Hcy,FA,VitB12 in early and middle pregnancy gestational weeks have been disordered.Serum Hcy,FA,VitB12 have a certain correlation with the progress of PE.
