血府逐淤汤加减对瘀阻脑络型血管性头痛51例脑血流动力学的影响

摘　　要：目的:探讨血府逐淤汤加减对血管性头痛(瘀阻脑络型)患者脑血流动力学指标的影响。方法:选择宁河区中医医院2019年10月至2020年12月收治的血管性头痛(瘀阻脑络型)患者102例,根据随机数字表法将患者分为两组,各51例。对照组采用尼莫地平治疗,试验组在此基础上加用血府逐淤汤加减治疗。治疗12周,比较两组患者临床疗效、疼痛程度、脑血管血流动力学指标[大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)的收缩期脑血流速度(Vs)]。结果:治疗后,试验组治疗总有效率较对照组高,差异有统计学意义(P<0.05);试验组VAS评分,ACA、PCA的Vs、MCA检测结果均较对照组低,差异有统计学意义(P<0.05)。结论:血府逐淤汤加减可提高血管性头痛(瘀阻脑络型)患者临床疗效,缓解疼痛程度,改善脑血流动力学状态。Objective:To investigate the effect of xuefu zhuyu decoction on cerebral hemodynamic indexes in patients with vascular headache(blood stasis blocking brain collateral type).Methods:102 patients with vascular headache(stasis of brain collateral type)admitted to Ninghe District Hospital of Traditional Chinese Medicine from October 2019 to December 2020 were selected and divided into two groups according to random number table method,51 cases in each group.The control group allowed nimodipine treatment,on this basis,the experimental group added xuefu zhuyu decoction treatment.After 12 weeks of treatment,clinical efficacy,pain degree and cerebral hemodynamic indexes of middle cerebral artery(MCA),anterior cerebral artery(ACA)and posterior cerebral artery(PCA)in systolic cerebral blood flow(Vs)were compared between the two groups.Results:After treatment,the total effective rate of experimental group was higher than that of control group,with statistical difference(P<0.05).VAS score,Vs and MCA test results of ACA and PCA in experimental group were lower than those in control group,with statistical differences(P<0.05).Conclusion:Xuefu zhuyu decoction can improve the clinical effect of vascular headache,relieve the pain degree and improve the cerebral hemodynamic state.
