摘  要:目的探讨术前血尿素氮(blood urea nitrogen,BUN)与左心室射血分数(left ventricular ejection fraction,LVEF)比值对非体外循环冠状动脉旁路移植术(off-pump coronary artery bypass grafting,OPCAB)后疗效的影响。方法回顾性分析作者医院2017-06/2019-10月在作者医院行OPCAB手术患者287例,以受试者工作特征(receiver operating characteristic,ROC)曲线计算临界值将患者分为低比值组(n=242)和高比值组(n=45),采用Cox回归分析患者发生主要不良心血管事件(major adverse cardiac events,MACE)的危险因素。结果单因素分析显示,手术时间、BUN、LVEF、BUN/LVEF比值与MACE发生率有关。多因素分析表明,BUN/LVEF比值(HR=1.058,95%CI:1.014~1.093,P<0.05)是发生MACE的预测因子。结论BUN/LVEF比值是预测OPCAB患者术后MACE事件发生率的预测因子。Objective To investigate the effects of preoperative blood urea nitrogen(BUN)and left ventricular ejection fraction(LVEF) ratio on the efficacy of off-pump coronary artery bypass grafting(OPCAB). Methods A total of 287 patients who underwent OPCAB operation in author′s hospital from June 2017 to October 2019 were retrospectively analyzed. The patients were divided into low ratio group(n=242) and high ratio group(n=45) according to critical valuecalculated by receiver operating characteristic(ROC) curve, the risk factors of major adverse cardiac events(MACE) in patients were analyzed by Cox regression analysis. Results Univariate analysis showed that the time of operation, the ratio of BUN, LVEF and BUN/LVEF were related to the incidence of MACE. And multivariate analysis showed that BUN/LVEF ratio(HR=1.058,95%CI:1.014-1.093,P<0.05) was a predictor of MACE. Conclusion BUN/LVEF ratio is a predictor of postoperative MACE events in patients with OPCAB.
