冠心病患者疾病相关指标水平变化及其与冠脉病变程度的相关性探讨

摘　　要：目的探讨冠心病患者疾病相关指标水平变化及其与冠脉病变程度的相关性。方法回顾性选取本院收治的冠心病患者80例,分析整理临床资料,依据Gensini评分将其分为低危组30例,中危组25例,高危组25例,另选取同期于本院进行健康体检的人员80例作为非冠心病组。所有研究对象均进行血浆Apo AI/Apo B、Hcy、hs-CRP水平、baPWV及血脂指标水平的检测,比较各组上述指标的水平变化情况,并分析冠脉病变程度与相关指标水平的相关性。结果较非冠心病组比,冠心病组血浆Apo AI/Apo By水平较低,血浆Hcy、血清LDL-C、TC、TG、hs-CRP水平、baPWV值则较高;高危组血浆Apo AI/Apo B水平低于低危组,血清TC、TG水平则高于低危组,高危组血浆Hcy、血清LDL-C、hs-CRP水平、baPWV值均高于中危组,且中危组高于低危组;冠心病患者Gensini评分与血清Apo AI/Apo B水平呈负相关,与血清Hcy、hs-CRP、LDL-C水平、baPWV均呈正相关(均P<0.05)。结论血清Apo AI/Apo B水平越低,血清Hcy、hs-CRP、LDL-C水平、baPWV则越高,冠心病患者冠脉病变程度越严重,预后情况越差。Objective To explore the changes of disease-related indexes in patients with coronary heart disease(CHD),and explore the correlation of degree of coronary artery lesion.Methods 80 patients with CHO admitted to our hospital were retrospectively selected,and were divided into three groups which were low-risk group for 30 cases,medium-risk group for 25 cases,and high-risk group for 25 cased according to Gensini score.Another 80 patients who underwent physical examination in our hospital during the same period were selected as non-CHD group.The levels of plasma Apo AI/Apo B,Hcy,hs-CRP,baPWV and blood lipid indexes were detected in all subjects,and the changes of these indexes in each group were compared,and the correlation between the degree of coronary artery disease and related indexes was analyzed.Results Compared with the non-CHD group,the plasma Apo AI/Apo by levels in CHD group were lower,the plasma Hcy,serum LDL-C,TC,TG,hs-CRP levels and baPWV values were higher.The level of plasma Apo AI/Apo B in high-risk group was lower than low-risk group,the levels of serum TC and TG were higher than low-risk group.The levels of plasma Hcy,serum LDL-C,hs-CRP and baPWV in high-risk group were higher than middle-risk group,and the middle-risk group was higher than low-risk group.Gensini score of patients with CHD negatively correlated with serum Apo AI/Apo B level,and positively correlated with serum Hcy,hs-CRP,LDL-C level and baPWV(all P<0.05).Conclusion The lower the serum Apo AI/Apo B level,the higher the serum Hcy,hs-CRP,LDL-C level and baPWV level,the more severe the degree of CHD.
