北京市某三级医院2010年-2019年住院分娩病例分析

摘　　要：目的 分析北京某三级医院2010年-2019年住院分娩病例,促进产科医疗质量的提升。方法 对2010年1月1日至2019年12月31日的31 155例住院分娩病例进行回顾性分析,按照分娩年龄是否≥35岁将分娩病例分为高龄组与适龄组,以2010年的数据为基期,计算报告期的定基发展速度,进行分娩量、分娩方式、并发症以及资源消耗等方面的比较。结果 2010年-2019年期间,分娩高峰分别在2014年及2016年,高龄产妇比例逐渐增加,剖宫产率逐渐下降,由59.82%降至43.15%,高龄组剖宫产率高于适龄组剖宫产率(χ^(2)=76.8770,P<0.0001)。高龄组妊娠期高血压、妊娠期糖尿病、前置胎盘及子痫发病率分别是7.80%、28.48%、2.20%及4.60%,均高于低龄组,差异有统计学意义。10年间平均住院日无下降趋势,平均住院费用呈上升趋势。结论 10年间高龄产妇比例逐渐增加,剖宫产率逐渐下降,产科医疗质量不断提升,应继续加强高龄孕妇的全程管理,不断降低剖宫产率,缩短平均住院日,保障母婴安全。Objective To analyze hospital delivery cases in a hospital from 2010 to 2019 and promote the improvement of obstetric medical quality.Methods A retrospective analysis of 31155 hospitalized delivery cases from January 1,2010 to December 31,2019.According to whether the delivery age is ≥35 years old or not,the delivery cases are divided into the elderly group and the appropriate age group.Use 2010 data as the base period to calculate the fixed-base development speed during the reporting period.Delivery volume,delivery mode,complications and resource consumption are compared between the two groups.Results From 2010 to 2019,the peaks of delivery were in 2014 and 2016.The proportion of advanced maternal age gradually increased,and the cesarean section rate gradually decreased,from 59.82% to 43.15%.The cesarean section rate in the elderly group The cesarean section rate was higher than that in the age group(χ^(2)=76.8770,P<0.0001).The incidences of hypertension,diabetes,placenta previa and eclampsia during pregnancy in the elderly group were 7.80%,28.48%,2.20% and 4.60%,which were higher than those in the younger group,and the difference was statistically significant.The average length of hospital stay in the past 10 years has not shown a downward trend,and the average hospitalization expenses have shown a significant upward trend.Conclusions In the past 10 years,the proportion of elderly group has gradually increased,the cesarean section rate has gradually decreased,and the quality of obstetric care has been continuously improved.The hospital should continue to strengthen the management of elderly pregnant women,continuously reduce the cesarean section rate,shorten the average hospital stay,and ensure the safety of mothers and babies.
