摘  要:目的 分析经面罩机械通气联合呼吸锻炼对慢性阻塞性肺疾病患者呼吸循环功能的影响,改善患者呼吸循环功能,促进肺功能恢复。方法 选择2018年2月1日-2020年2月28日在某院住院治疗的140例COPD稳定期患者,根据随机数字表法将其分为两组,对照组70例予以经面罩机械通气治疗,观察组70例予以经面罩机械通气联合呼吸锻炼治疗,7天后对比两组患者的呼吸循环功能、血气分析指标及肺功能。结果 观察组治疗7天后的呼吸频率为(28.86±3.06)次/分,心率水平为(101.26±5.19)次/分,比对照组的(30.11±2.95)次/分及(110.38±5.22)次/分更低(P<0.05);观察组治疗后动脉血氧分压为(57.35±6.15)mmHg,动脉血氧饱和度水平为(90.12±5.33)%,比对照组的(52.28±6.20)mmHg及(86.85±5.67)%高,而观察组的动脉二氧化碳分压水平为(62.16±5.35)mmHg,比对照组的(70.19±6.91)mmHg低(P<0.05);观察组治疗后最大呼气中段平均流速、第一秒用力呼吸容积/用力肺活量、呼气峰流速值及FEV1占预测值百分比均比对照组高(P<0.05)。结论 经面罩机械通气联合呼吸锻炼治疗COPD稳定期患者的效果显著,有助于改善其呼吸循环功能及血气分析指标,并且促进肺功能恢复。Objective To analyze the effect of breathing exercise combined with mechanical ventilation through face mask on respiratory and circulatory function of patients with COPD,improve the respiratory and circulatory function,promote the recovery of lung function.Methods Random sample of 140 cases of patients with stable COPD who were hospitalized in a Hospital from February 1,2018 to February 28,2020 were collected,randomly divided into two groups according to the random number table method.70 cases in the control group were treated with mechanical ventilation through face mask,70 cases in the observation group were treated with breathing exercise combined with mechanical ventilation through face mask.The respiratory and circulatory function,blood gas analysis indexes and lung function were compared between the two groups after 7 days.Results In the observation group,the respiratory rate after 7 days of treatment was(28.86±3.06) breaths/min,and the heart rate level was(101.26±5.19) breaths/min,which were lower than the control group’s(30.11±2.95) breaths/min and(110.38±5.22) breaths/min(P<0.05).In the observation group,the arterial oxygen partial pressure after treatment was(57.35±6.15) mm Hg,and the arterial oxygen saturation level was(90.12±5.33)%,which were higher than(52.28±6.20) mmHg and(86.85±5.67)% in the control group.The arterial partial pressure of carbon dioxide in the observation group was(62.16±5.35) mmHg,which was lower than the control group’s(70.19±6.91) mmHg(P<0.05).In the observation group,the MMEF,FEV1/FVC,PEF and FEV1% after treatment were higher than in the control group(P<0.05).Conclusion Breathing exercise combined with mechanical ventilation through a face mask has a significant effect in the treatment of patients with stable COPD,which can help to improve their respiratory and circulatory functions and blood gas analysis indicators,as well as promote the recovery of lung function.
