摘　　要：目的 分析某市三级综合医院医疗质量现状与趋势,为进一步提高区域医院医疗质量提供参考。方法 以某市8家三级综合医疗机构2016年1月1日-2019年12月31日共216.6911万例住院病案首页为研究对象,在参考资料与结合数据的基础上选取包括医疗能力、医疗效率和医疗效果3方面共9项体现医疗质量的主要指标,运用AHP层次分析法和TOPSIS法对其医疗质量进行综合评价。结果 2016年-2019年2种评价方法结果均显示目标医院的医疗质量综合评价得分逐年升高:2019年的医疗质量评价结果最好,综合评分CIi值为0.9669,Ci值为0.6974;2016年的医疗质量评价结果最差,CIi值为0.9186,Ci值为0.3745。2016年-2019年8家三级综合医院住院治疗需求量增加,医疗能力和效率显著提升,减费降费效果显现。结论 某市8家三级综合医院医疗质量逐步改善,基于住院病案首页可获得性数据并综合运用AHP法和TOPSIS法可在一定程度上反映医院医疗质量情况,具有客观性和准确性。Objectives To analyze the current situation and trend of medical quality in a tertiary general hospital in a city,so as to provide reference for further improving the medical quality of regional hospitals.Methods From January 1st,2016 to December 31st,2019 in 8 tertiary comprehensive medical institutions in a city,a total of 2,166,911 inpatient records were selected as the research object.On the basis of the references and combining with the data selected the three aspects(medical ability and efficiency of medical and health care effect) a total of nine main indicator of medical quality,using The AHP hierarchical analysis method and TOPSIS method to comprehensive evaluation of the medical quality.Results From 2016 to 2019,both evaluation methods showed that the comprehensive evaluation score of the target hospital increased year by year:the best medical quality evaluation result in 2019,with CIi of 0.9669,Ci of 0.6974,and the worst medical quality evaluation result in 2016,with the comprehensive score CIi of 0.9186 and Ci of 0.3745.In 2016-2019,the demand for hospitalization of the eight tertiary general hospitals increased,the medical capacity and efficiency improved significantly,and the effect of fee reduction and fee reduction showed.Conclusions From 2016 to 2019,the medical quality of 8 tertiary general hospitals in a city were gradually improved.Based on the data of the availability of the front page of inpatient medical records and the comprehensive application of AHP and TOPSIS,the medical quality of hospitals could be reflected to some extent,with objectivity and accuracy.
