摘　　要：目的 建立某三甲综合医院的多维度医疗质量指标评价体系,综合评价某院医疗质量,为医院管理工作提供参考依据。方法 选择TOPSIS法和综合指数法,从DRGs评价指标、工作强度、工作效率、诊断质量4个方面对医院2016年1月1日-2020年12月31日259 643份病案数据进行医疗质量综合评价和分析。采用Pearson等级相关检验对TOPSIS法和综合指数法的评价结果进行相关性分析。结果 TOPSIS法和综合指数法评价结果显示该院医疗质量呈逐年上升趋势,均为2020年最佳,2016年最差。TOPSIS法评价结果显示2016年-2020年Ci值分别为0.2439、0.2544、0.4692、0.6800、0.7769,综合指数法评价结果显示2016年-2020年综合指数值分别为3.4036、3.6030、3.9339、4.4758、4.9820。2种方法经等级相关系数法检验,r=0.9,P<0.05,具有高度一致性和相关性。结论 基于TOPSIS法与综合指数法综合评价DRGs指标、工作强度、工作效率、诊断质量4个维度可为医院规范化管理提供数据支持。综合评价结果显示医院近5年来医疗质量呈逐年上升趋势。Objectives This research establish a medical quality index evaluation system of a Three A Grade General Hospital based on multi-dimensional comprehensive,and comprehensively evaluate the medical quality of the hospital,so as to provide a reference for hospital management.Methods TOPSIS method and comprehensive index method were selected to evaluate and analyze the medical quality of 259 643 medical records from January 1st,2016 to December 31st,2020 from four aspects:work intensity,work efficiency,diagnosis quality and DRGs index.In this study,Pearson correlation analysis was used to analyze the evaluation results of TOPSIS method and synthetic index method.Results The evaluation results of TOPSIS method and comprehensive index method show that the medical quality of the hospital was increasing year by year,which was the best in 2020 and the worst in 2016.The evaluation results of TOPSIS method show that the CI values from 2016 to 2020 were 0.2439,0.2544,0.4692,0.6800 and 0.7769 respectively.The evaluation results of comprehensive index method show that the comprehensive index values from 2016 to 2020 were 3.4036,3.6030,3.9339,4.4758 and 4.9820 respectively.The results of the two evaluation methods were consistent(r =0.9,P<0.05).Conclusions Based on TOPSIS method and comprehensive index method,the four dimensions of DRGs index,work intensity,work efficiency and diagnosis quality were comprehensively evaluated to provide data support for the standard management of hospitals.The comprehensive evaluation results showed that the medical quality of the hospital had increased year by year in recent 5 years.
