某三甲妇幼医院出院患者疾病构成分析

摘　　要：目的 分析妇幼医院出院患者的变化趋势和疾病构成,为妇幼医院的管理决策与专科建设提供参考依据。方法 收集某院2016年1月1日-2020年12月31日出院患者数据,对总计269394例出院患者住院病案首页疾病主要诊断进行疾病分类和回顾性分析。结果 出院患者呈逐年上升趋势,出院患者主要集中在产科和妇科,占比分别为61.08%和22.14%。妊娠、分娩和产褥期疾病、起源于围生期某些情况的疾病、泌尿生殖系统疾病这3类疾病出院患者人数构成比的总和约为83.33%。妊娠合并瘢痕子宫、胎膜早破(在24小时之内产程开始)、新生儿高胆红素血症、妊娠期糖尿病、支气管肺炎和新生儿肺炎等疾病的构成比较高,新生儿高胆红素血症疾病呈逐年上升趋势。结论 妇幼医院应以构成比高的疾病作为疾病防治工作重点,加强监控发病率上升的疾病,加大宣传疾病防范意识。Objectives To understand the changing trends and disease composition of discharged patients,and provide basis for the management and development of the Maternity and Children’s Hospital.Methods Data of discharged patients from 2016 to 2020 were collected,and the disease analysis were performed based on the diagnosis on the front page of the medical records of 269,394 discharged patients.Results The number of discharged patients increased year by year.The discharged patients in obstetrics and gynecology were accounting for 61.08%and 22.14%,respectively.Pregnancy,childbirth and puerperium diseases,perinatal diseases,and urogenital diseases,these three types of diseases accounted for 83.33% of the discharged patients.Pregnancy with scarred uterus,premature rupture of membranes(the process begins within 24 hours),neonatal hyperbilirubinemia,gestational diabetes,bronchial pneumonia,neonatal pneumonia and other diseases accounted for a relatively high proportion.Neonatal hyperbilirubinemia disease was on the rise,and the type D cases among discharged patients with neonatal pneumonia were much higher than that of other diseases.Conclusions The Maternity and Children’s Hospital should focus on the development of obstetrics and gynecology,and strengthen the treatment of diseases with high proportion and monitor the diseases with increased incidence,and furtherly increase awareness of disease prevention to reduce the occurrence of diseases.
