胫骨骨缺损纵向骨搬移术的手术编码分析

摘　　要：为提高用于治疗胫骨骨缺损的纵向骨搬移术的手术编码的质量,为医保付费、临床研究等做好数据支撑。根据国际疾病分类手术与操作ICD-9-CM-3的分类要求、通过与临床医师沟通交流、查阅纵向骨搬移术相关文献了解手术目的及具体方式、并结合案例分析等多种方式,对该手术编码进行探讨。完整的纵向骨搬移术治疗胫骨骨缺损的手术编码除了胫骨延长术78.37,胫骨外固定术78.17,外部固定装置(环型系统)84.72,通常还附有胫骨病损切除术77.67和骨骼牵引93.44的编码。因ICD-9-CM-3没有该手术的明确分类,编码人员需不断深入了解临床知识结构,才能确保编码质量,做好数据基础。In order to improve the quality of surgical coding for longitudinal bone removal for the treatment of tibia defects,data support is provided for medical insurance payment,clinical research,etc.According to the classification requirements of ICD-9-CM-3 for international classification surgery and operation,the surgical code is discussed by communicating with clinicians,consulting the literature related to longitudinal bone transplantation to understand the purpose and specific methods of surgery,and combining case studies.The complete surgical code for the treatment of tibia bone defects by longitudinal bone transplantation is 78.37,the outer tibia fixation 78.17,the external fixation device(ring system) 84.72,usually accompanied by the code of tibia loss excision 77.67 and bone traction 93.44.Since ICD-9-CM-3 does not have a clear classification of the operation,coders need to continuously understand the clinical knowledge structure in order to ensure the quality of coding and do a good job of data base.
