脊髓损伤患者急性应激障碍及其影响因素研究

摘　　要：目的 探讨脊髓损伤患者急性应激障碍现状及影响因素,为预防和改善脊髓损伤患者的急性应激障碍状况提供依据。方法 选取2016年5月10日-2019年12月10日某院脊髓损伤住院患者264例,采用斯坦福急性应激反应问卷、疼痛视觉模拟评分尺、医院焦虑抑郁量表和一般人口学资料对患者进行评测。结果 264例脊髓损伤患者急性应激障碍总分均值为(78.29±41.341)分,总分≥40分者125例,急性应激障碍发生率47.34%;疼痛评分、神经损伤平面、脊髓损伤分级、焦虑评分、抑郁评分进入急性应激障碍总分回归方程,重度疼痛、颈椎损伤、脊髓损伤分级为A级、焦虑、抑郁患者在急性应激障碍总分及各个维度评分较高,差异有统计学意义(P<0.05)。结论 脊髓损伤患者存在明显的急性应激反应,脊髓损伤分级为A级、颈椎损伤、重度疼痛、焦虑、抑郁的患者发生急性应激障碍的可能性更大。Objectives To investigate the status and influencing factors of acute stress disorder in patients with spinal cord injury,and provide evidence for the prevention and improvement of acute stress disorder in patients with spinal cord injury.Methods A total of 264 inpatients with spinal cord injury in a hospital were selected from May 10th,2016 to December 10th,2019.The patients were evaluated by Stanford Acute Stress Response Questionnaire,visual analog scale of pain,hospital anxiety and depression Scale and general demographic data.Results The mean score of acute stress disorder in 264 patients with spinal cord injury was(78.29±41.341)points,125 patients with total score over 40 points,and the incidence of acute stress disorder was 47.34%.Pain score,nerve injury,plane and grade of spinal cord injury,anxiety score,depression score into the regression equation of the acute stress disorder total score,severe pain,cervical vertebra injury,spinal cord injury classification into class A,acute stress disorder in patients with anxiety,depression,total score and each dimension score is higher,the difference was statistically significant(P<0.05).Conclusions Spinal cord injury patients had obvious acute stress reaction,spinal cord injury grade A,cervical spine injury,severe pain,anxiety,depression patients were more likely to develop acute stress disorder.
