天津市48家公立医院中药配方颗粒应用管理现况调查

摘　　要：目的 随着中国经济的快速发展、各级政府的政策支持,中药配方颗粒得到了极快的发展,本研究旨在了解当前中药配方颗粒在医院临床应用各环节中存在的管理问题,为完善中药配方颗粒管理体系提供依据。方法 选取天津市48所公立医院药剂科负责人进行现场问卷调查,调查工具为“中药配方颗粒管理现况调查问卷”。结果 48所医院中,36所医院使用中药配方颗粒,占75.00%,月平均销售额为(44.39±12.23)万元,单张处方金额平均(536.72±26.20)元。中药配方颗粒处方在28所医院由中医、西医医师开具,占77.78%,在24所医院由西药师调剂,占66.67%,22所医院由西药师核验,占61.11%,29所医院由中药师点评,占80.56%,20所医院由西药房采买、验收,占55.56%,23所医院由西药房储存、养护中药配方颗粒,占63.89%,中药配方颗粒的管理情况在不同级别医院间比较,差异无统计学意义(P> 0.05)。结论 中药配方颗粒已经成为天津市公立医院药房品类的主体,但在应用过程中,中药配方颗粒的定位尚不清晰,管理存在混乱,有待进一步明确管理标准。Objective The purpose of this study is to investigate the status of the management problems existing in the clinical application of TCM formula granules in medical institutions,provide the basis for improving the management system of TCM formula granules.Methods A questionnaire survey was conducted on the heads of pharmacy departments in 48 public medical institutions in Tianjin by random sampling method,the survey tool was a questionnaire on the management status of TCM formula granules.Results Of the 48 hospitals,36 used TCM formula granules,accounting for 75%,average monthly sales were(443.9±122.3) thousand yuan,the average amount of a single prescription was(536.72±26.20) yuan.The prescription of TCM formula granules were issued by doctors of TCM and Western medicine in 28 hospitals,accounting for 77.78%,were dispensed by western pharmacists in 24 hospitals,accounting for 66.67%,were inspected by western pharmacists in 22 hospitals,accounting for 61.11%,were reviewed by Chinese pharmacists in 29 hospitals,accounting for 80.56%,were purchased and accepted by Western Pharmacy in 20 hospitals,accounting for 55.56%,were stored and maintained by the Western Pharmacy in 23 hospitals,accounting for 63.89%.There was no statistical significance in the management of TCM formula granules among hospitals of different levels.Conclusion Traditional Chinese Medicine granule has become the main body of pharmacy category in public medical institutions in Tianjin.However,in the process of application,the positioning of TCM formula granules particles was not clear.There was confusion in management confusion.Management standards needed to be further clarified.
