超声引导下经皮肾活检术的临床应用价值及并发症分析

摘　　要：目的探讨超声引导下经皮肾活检(PRB)术的临床应用价值、并发症及相关危险因素,为肾脏活检相关决策提供依据。方法收集2020年9月至2021年3月在芜湖市第二人民医院行超声引导下PRB术患者共236例,其中男120例,女116例,依据术后有无并发症分为有并发症组和无并发症组,分析患者临床资料,评估并发症的危险因素。结果本研究取材成功率为100%,均满足病理诊断需求。术后出现并发症20例(8.47%),单因素分析显示,两组收缩压、血红蛋白计数、血肌酐值比较,差异有统计学意义(P<0.05)。多因素分析显示,血红蛋白计数和血肌酐值是并发症的影响因素(P<0.05)。结论超声引导下PRB术安全高效,血红蛋白计数降低、血肌酐值升高是并发症的危险因素。Objective To investigate the clinical application value,complications,and correlative risk factors of ultrasound-guided percutaneous renal biopsy,so as to provide a basis for decision-making related to renal biopsy.Methods A total of 236 patients who underwent ultrasound-guided percutaneous renal biopsy in the Second People’s Hospital in Wuhu City from September 2020 to March 2021 were collected,including 120 males and 116 females.They were divided into the complications group and the non-complication group according to whether there were postoperative complications.The clinical data of patients were analyzed,and the risk factors of complications were evaluated.Results The success rate of this research was 100%,all of which met the pathological diagnosis needs.Postoperative complications occurred in 20 cases(accounting for 8.47%).Univariate analysis showed that there were statistically significant differences in systolic blood pressure,hemoglobin count and serum creatinine between the two groups(P<0.05).Multivariate analysis showed that hemoglobin count and serum creatinine were the impacting factors of complications(P<0.05).Conclusion Ultrasound-guided percutaneous renal biopsy is safe and highly efficient.The decrease of hemoglobin count and the increase of serum creatinine are risk factors of complications.
