县域医共体内三级综合康复管理模式分级推广及应用实践

摘　　要：目的探讨医共体模式下适合县域三级医院康复规范化诊疗技术的推广运行和管理模式,促进构建脑卒中慢病医防结合新融合,加快推进医共体建设。方法选取2020年3月至2021年3月于嘉善县第一人民医院医共体内就诊的200例脑卒中患者进行研究,按照随机数字表法将其分成对照组和观察组,每组各100例。对照组实施常规康复管理,如三级康复干预。观察组在对照组基础上实施县域医共体内三级综合康复管理模式。于管理后4个月评价两组医疗质量和患者康复效果,调查两组患者对康复管理模式的满意度。结果观察组患者管理、康复管理、健康宣教评分明显优于对照组,差异有统计学意义(P<0.05)。两组患者干预后各项功能均明显优于干预前,差异有统计学意义(P<0.05),观察组患者干预后康复效果(日常生活活动能力、运动功能、生活质量)明显优于对照组,差异有统计学意义(P<0.05)。观察组患者对康复管理模式的满意度明显高于对照组,差异有统计学意义(P<0.05)。结论县域医共体内三级综合康复管理模式不仅可提升县域医院管理质量,同时还可提高患者康复效果,实现医患双赢的局面,值得推广。Objective To explore a promotion and management model of standardized rehabilitation technology suitable for county-level tertiary hospitals under the medical community model,so as to promote a new integration of treatment and prevention for chronic stroke and accelerate the construction of medical community.Methods A total of 200 stroke patients attending the medical community of the First People’s Hospital of Jiashan from March 2020 to March 2021 were selected as study subjects,and were randomly divided into the control group and the observation group according to the number method,with 100 patients in each group.The control group received conventional rehabilitation management,such as tertiary rehabilitation intervention,while the observation group received a tertiary integrated rehabilitation management model within the county medical community on the basis of the management model in the control group.The medical quality and the rehabilitation effect of patients in both groups were evaluated at 4 months after management,and the satisfaction of patients in both groups with the rehabilitation management model was investigated.Results The scores in patient management,rehabilitation management and health education in the observation group were significantly higher than those in the control group,with statistically significant differences(P<0.05).After intervention,all functions of patients in both groups were significantly better than those before intervention,with statistically significant differences(P<0.05).The rehabilitation effect(activities of daily living,motor function and quality of life)of patients in the observation group was significantly better than that of the control group after 4 months of intervention,with statistically significant difference(P<0.05).The satisfaction of patients in the observation group with the rehabilitation management model was significantly higher than that in the control group,with statistically significant difference(P<0.05).Conclusion The ter tiary in tegrated rehabilita
