摘　　要：目的评价溴吡斯的明(PB)与可乐定单日联合激发试验对儿童生长激素缺乏症(GHD)的诊断价值。方法采用前瞻性研究,选取2018年6月至2020年6月潍坊市人民医院小儿内科就诊的矮小症患儿120例,采用随机数字表法分成两组,试验组(n=61)采用PB与可乐定单日联合激发试验,对照组(n=59)采用经典的胰岛素与可乐定两日序贯激发试验,采用化学发光免疫法检测生长激素(GH),比较两种方法GH缺乏的阳性率及不良反应发生率,以评估该GH激发试验的有效性和安全性。结果两组一般资料、激发试验GH峰值及胰岛素样生长因子-1(IGF-1)水平比较,差异无统计学意义(P>0.05)。两组GH激发试验阳性率和不良反应阳性率比较,差异均无统计学意义(P>0.05)。结论PB与可乐定单日联合激发试验和经典胰岛素耐量试验(ITT)得出的结果具有很高的一致性,可以用于儿童GHD的诊断,是一种简单、有效的试验方法。Objective To evaluate the diagnostic significance of single-day combined stimulation test of pyridostigmine bromide(PB)and clonidine in children with growth hormone deficiency(GHD).Methods A prospective research was adopted.A total of 120 children with dwarfism admitted to the Department of Pediatric Internal Medicine in Weifang People’s Hospital from June 2018 to June 2020 were selected,and they were divided into the experimental group and the control group according to the random number table method.The experimental group(n=61)was treated with single-day combined stimulation test with PB and clonidine,while the control group(n=59)was treated with conventional two-day sequential stimulation test with insulin and clonidine.Growth hormone(GH)was detected by chemiluminescent immunoassay.The positive rate of GHD and the incidence of adverse reactions(ADRs)between the two methods were compared to evaluate the effectiveness and safety of the GH stimulation test.Results There were no statistically significant differences i n general data,GH peak of the stimulation test and insulin-like growth factor-1(IGF-1)level between the experimental group and the control group(P>0.05).And there were no statistically significant differences in the positive rate and the incidence of ADRs of the stimulation test between the two groups(P>0.05).Conclusion The results obtained by single-day combined stimulation test and conventional insulin tolerance test(ITT)with PB and clonidine are highly consistent.It can be used for diagnosis of children with GHD.Therefore,it is a simple and effective experimental method.
