摘 要:目的 探讨绝经后代谢综合征(MS)各组分与骨量下降的相关性.方法 选择2017年6月至2020年6月符合标准的绝经后MS患者186例,根据双能X线骨密度仪测定的骨密度T值,分为骨量正常组33例、骨量减少组56例、骨质疏松组97例,记录所有患者年龄、绝经年限、生育次数、分娩方式、文化程度、饮酒史、规律运动、钙剂补充、既往骨折史等基础情况,以及BMI、收缩压、舒张压、FPG、TG、LDL-C、HDL-C水平.比较三组间患者基础情况及代谢指标,有序Logistic回归分析绝经后MS患者各组分与骨量下降的相关性.结果 三组患者年龄、绝经年限、文化程度、规律运动、钙剂补充、既往骨折史及收缩压、FPG、TG、LDL-C、HDL-C水平差异有统计学意义(P<0.05).校正基础情况因素,Logistic回归分析结果显示:高收缩压(β=0.025,P=0.027)、高FPG(β=0.260,P=0.022)、高TG(β=1.536,P=0.003)及低HDL-C(β=-1.801,P=0.009)是引起绝经后代谢综合征患者骨量下降的独立危险因素.结论 高水平收缩压、高FPG、高TG及低水平HDL-C是加剧绝经后MS患者骨量下降的重要因素,故对于绝经后MS患者,应进行规律运动锻炼,积极控制血压、血糖及血脂水平,并定期监测,延缓骨量下降.Objective To observe the correlation between components of postmenopausal metabolic syndrome(MS)and bone loss.Methods 186 postmenopausal women with metabolic syndrome were selected from June 2017 to June 2020,and were divided into normal bone mass group(33 cases),bone mass flling group(56 cases)and osteoporosis group(97 cases)according to T scores of BMD measured by dual energy X-ray bone density instrument.Basic data such as age,menopausal years,number of births,mode of delivery,educ ational level,drinking history,regular exercise,calcium supplerment and previous fracture history were recorded,and also the levels of BMI,systolic pressure,diastolic pressure,FPG,TG,LDL-C and HDL-C.The basic data and metabolic index of the patients were compared among the three groups.Ordinal Logistic regression further analyzed the correlation between the metabolic level of each component and bone loss in postmenopausal patients with metabolic syndrome.Results There were significant differences in age,menopausal years,educational level,regular exercise,calcium supplement,previous fracture history,systolic pressure level,FPG;level,TG level,LDL-C level and HDL-C level between the three groups(P<0.05).Adjusted for the Basic data,Logistic regression analysis showed that high level systolic blood pressure(β=0.025,P=0.027),High level FPG(β=0.260,P=0.022),High level TG(β=1.536,P=0.003)and low level HDL-C(β=-1.801,P=0.009)were independent risk factors for bone loss in postmenopausal patients with metabolic syndrome.Conclusion High level systolic blood pressure,High level FPG,High level TG and low level ofHDL-C are the important factors that aggravate the loss of bone mass in postmenopausal patients with metabolic ayndrome.Therefore,for postmenopausal patients with metabolic syndrome,regular exercise should be carried out,and actively control blood pressure,blood glucose and blood lipid levels,so as to delay the loss of bone mass.
