摘　　要：目的探讨心脏彩超对高血压左心室肥厚伴左心衰竭的诊断效果及准确性。方法择取2019年3月—2020年5月至福建医科大学附属漳州市医院就诊的高血压左心室肥厚伴左心衰竭患者共计120例设置为试验组,另同期选择120例健康体检人群设置为对照组,对两组受检者均进行心脏彩超检查,试验组患者进行心功能分级,对比其心脏彩超指标,分析心脏彩超的诊断准确性。结果试验组的左室射血分数相比对照组更低,左室重量、左室质量指数、左室舒张末内径、左室短轴缩短均高于对照组,差异有统计学意义(P<0.05);心功能Ⅱ级患者中,左室舒张末内径(left ventricular end-diastolic diameter,LVEDD)、左室重量(left ventricular mass,LVM)、左室短轴缩短(left ventricular fractional shortening,LVFS)、左室质量指数(left ventricular mass index,LVMI)、检测水平均明显低于心功能Ⅲ~Ⅳ级患者,左室射血分数(left ventricular ejection fraction,LVEF)水平明显高于心功能Ⅲ~Ⅳ级患者,说明试验组心功能分级Ⅱ级、Ⅲ~Ⅳ级的心脏彩超指标比较差异有统计学意义(P<0.05);心脏彩超对患者的检测准确率为92.50%(111/120)。结论对高血压左心室肥厚伴左心衰竭患者来说,心脏彩超检查具有一定的诊断价值,诊断准确率较高,可用于评估患者心功能,为后续的临床治疗提供相关的影像学资料,值得采纳。Objective To investigate the diagnostic effect and accuracy of cardiac color Doppler ultrasound on hypertensive left ventricular hypertrophy with left heart failure.Methods A total of 120 patients with hypertension,left ventricular hypertrophy and left heart failure who were treated in Zhangzhou Hospital Affiliated to Fujian Medical University from March 2019 to May 2020 were selected as the experimental group.In the same period,120 healthy patients were selected as the control group.All subjects in the group underwent cardiac color Doppler ultrasound examination,and the patients in the test group were rated for cardiac function,compared with their cardiac color Doppler ultrasound indexes,and analyzed the diagnostic accuracy of cardiac color Doppler ultrasound.Results The left ventricular ejection fraction of the experimental group was lower than that of the control group,and the left ventricular weight,left ventricular mass index,left ventricular end-diastolic inner diameter,and left ventricular short axis shortening were all higher than those of the control group,the differences were statistically significant(P<0.05).In patients with cardiac function gradesⅡ,the detection levels of LVEDD,LVM,LVFS,and LVMI were significantly lower than those in patients with cardiac function gradesⅢ~Ⅳ,and the LVEF level was significantly higher than that in patients with cardiac function gradesⅢ~Ⅳ,indicating in the experimental group,there was a statistically significant difference in the cardiac color Doppler ultrasound indexes of cardiac function gradesⅡandⅢ~Ⅳ(P<0.05).The detection accuracy of cardiac color Doppler ultrasound was 92.50%(111/120).Conclusion For patients with hypertensive left ventricular hypertrophy and left heart failure,cardiac color Doppler ultrasound examination has a certain diagnostic value,and the diagnostic accuracy is high.It can be used to assess the heart function of patients and provide relevant imaging data for subsequent clinical treatment.It is worthwhile adoption.
