膝关节骨性关节炎患者血清葡萄糖-6-磷酸异构酶、硫酸软骨素846、血小板反应蛋白-1水平检测及临床意义

摘　　要：目的探讨膝关节骨性关节炎(KOA)患者血清葡萄糖-6-磷酸异构酶(GPI)、硫酸软骨素846(CS846)、血小板反应蛋白-1(TSP-1)水平及临床意义。方法选取该院2019年3月至2020年11月收治的112例KOA患者作为研究对象,按照Kellgren-Lawrence(K-L)分级标准分为K-L 2级组42例、K-L 3级组33例、K-L 4级组37例,另选取同期在该院进行健康体检的志愿者50例作为对照组。比较各组血清GPI、CS846、TSP-1水平,并比较3组KOA患者西安大略麦克马斯特大学骨性关节炎调查量表(WOMAC)评分及血清C反应蛋白(CRP)、红细胞沉降率(ESR)及白细胞介素(IL)-17水平。采用Spearman相关和Pearson相关分析血清GPI、CS846、TSP-1水平与KOA患者K-L分级、WOMAC评分及血清CRP、ESR、IL-17水平之间的关系。结果K-L 2级、K-L 3级、K-L 4级组患者血清GPI、CS846、TSP-1水平均高于对照组,K-L 3级组患者高于K-L 2级组,K-L 4级组患者又高于K-L 3级组,差异均有统计学意义(P<0.05)。K-L 3级组和K-L 4级组患者各项WOMAC评分及总分均高于K-L 2级组,K-L 4级组患者又高于K-L 3级组,差异均有统计学意义(P<0.05)。K-L 3级组和K-L 4级组患者血清CRP、ESR、IL-17水平均高于K-L 2级组,K-L 4级组患者又高于K-L 3级组,差异均有统计学意义(P<0.05)。Spearman相关和Pearson相关分析结果显示,血清GPI、CS846、TSP-1水平与KOA患者K-L分级、WOMAC评分、血清CRP、ESR、IL-17水平均呈正相关(P<0.05)。结论KOA患者血清GPI、CS846、TSP-1水平均存在异常高表达,并且与KOA患者病情严重程度密切相关,可作为临床评估KOA患者病情严重程度的可靠指标。Objective To investigate the levels of serum glucose-6-phosphate isomerase(GPI),chondroitin sulfate 846(CS846)and thrombospondin-1(TSP-1)in patients with knee osteoarthritis(KOA)and their clinical significance.Methods A total of 112 patients with KOA admitted to the hospital from March 2019 to November 2020 were selected as the research subjects.According to the Kellgren-Lawrence(K-L)grading standard,they were divided into K-L grade 2 group with 42 cases,K-L grade 3 group with 33 cases,and K-L grade 4 group with 37 cases.Another 50 volunteers who underwent physical examination in the hospital during the same period were taken as the control group.Serum GPI,CS846,TSP-1 levels in each group were compared.The score of the McMaster University Osteoarthritis Survey Scale(WOMAC)and the serum C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and interleukin(IL)-17 levels in three groups of KOA patients were compared.Spearman correlation and Pearson correlation were used to analyze the relationship between the serum GPI,CS846,TSP-1 levels and K-L grade,WOMAC score,serum CRP,ESR and IL-17 levels in KOA patients.Results The serum levels of GPI,CS846 and TSP-1 in the K-L grade 2,K-L grade 3 and K-L grade 4 groups were higher than those in the control group,the K-L grade 3 group was higher than the K-L grade 2 group,and the K-L grade 4 group was higher than the K-L grade 3 group,the differences were statistically significant(P<0.05).The each WOMAC score and total scores in the K-L grade 3 group and the K-L grade 4 group were higher than those in the K-L grade 2 group,and the K-L grade 4 group was higher than the K-L grade 3 group,and the differences were statistically significant(P<0.05).The serum levels of CRP,ESR and IL-17 in the K-L grade 3 group and K-L grade 4 group were higher than those in the K-L grade 2 group,and the K-L grade 4 group was also higher than the K-L grade 3 group,and the differences were statistically significant(P<0.05).Spearman correlation and Pearson correlation analysis showed that serum G
