摘 要:目的:采用三维全自动左心容积定量技术(HM)评估阵发性房颤患者的左心功能并探讨其临床价值。方法:选取40例阵发性房颤患者使用双平面Simpson法、HM法和实时三维超声(RT-3DE)法分别评估左心室舒张末容积(LVEDV)、左心室收缩末容积(LVESV)以及左室射血分数(LVEF),并记录分析所用时间。将3种方式的测量结果以及分析所用时间进行比较并对HM法的可重复性进行检验。结果:HM法测量的LVEDV、LVESV以及LVEF相较于RT-3DE法差异无统计学意义(均P>0.05),双平面Simpson法测量的LVEDV和LVEF均高于HM法和RT-3D法的测量结果(均P<0.05),而3种方法测量的LVESV结果差异无统计学意义(均P>0.05)。HM法和RT-3D法之间有着很高的相关性和一致性,且HM法在3种方法中分析所用时间最短。重复性检验中HM法在观察者间和观察者内都有较高的重复性。结论:HM法是一种操作步骤简便,可以快速、准确地测量评估阵发性房颤患者左心功能的技术,且具有较高的可重复性。Objective:To evaluate the clinical value of Heart Model(HM)in left heart function among patients with paroxysmal atrial fibrillation.Methods:The 40 patients with paroxysmal atrial fibrillation were recruited and evaluated for left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV)and left ventricular ejection fraction(LVEF)by the biplane Simpson method,HM method and real-time three-dimensional echocardiography(RT-3DE)methods,respectively.The time taken for analysis was recorded.The measurement results of the three methods as well as the time taken for the analysis were compared.And the repeatability of HM method within and between groups was evaluated.Results:LVEDV,LVESV and LVEF measured by HM method compared with RT-3DE method were not statistically significant(P>0.05),LVEDV and LVEF measured by biplane Simpson's method were higher than those measured by HM method and RT-3DE method(P<0.05),while LVESV measured by three methods were not statistically significant(P>0.05).There was a high correlation and consistency between the HM method and the RT-3D method and the HM method had the shortest time used for analysis among the three methods.The repeatability of HM method was great within and between groups.Conclusions:The HM method is an easy procedure and provides rapid and accurate measurement of left heart function in patients with paroxysmal atrial fibrillation with excellent repeatability.In clinical work,it has great potential to shorten the examination and analysis time,reduce artificial error and improve the reliability of measurement results.
