摘　　要：海因里希金字塔关于严重伤害和死亡预防理论存在对员工过分责备,误导未遂事故和死亡事故关系等缺陷,介绍了关注严重伤害和死亡可能性的意义以及预防严重伤害和死亡可能性的具体做法,阐述了海因里希金字塔与严重伤害和死亡可能性的关系,通过一种新的金字塔理论定义了严重伤害与死亡的三个具体指标,并提出严重伤害与死亡的预防措施。Heinrich’s pyramid on the theory of serious injury and fatality prevention has flaws such as over-blaming employees and misleading the relationship between near misses and fatal accidents, the significance of focusing on the possibility of serious injury and fatality and specific practices to prevent the possibility of serious injury and fatality are introduced, the relationship between Heinrich’s pyramid and the possibility of serious injury and fatality is elaborated. Three specific indicators of serious injury and fatality by a new pyramid theory are defined, and preventive measures for serious injury and fatality are proposed.
