沙参麦冬汤联合西医治疗放射性肺炎临床观察

摘　　要：目的:探讨沙参麦冬汤联合西医治疗放射性肺炎(RP)的临床疗效。方法:将62例RP患者按照随机数字表法将其分为对照组和观察组各31例,对照组采用西医常规治疗,观察组患者在其治疗基础上采用沙参麦冬汤治疗。7d为1个疗程,治疗2个疗程后比较2组临床疗效、主要症状评分、实验室指标、RTOG分级评分和KPS评分。结果:相较于对照组,观察组治疗后干咳痰少、咽干口燥、气短乏力主要症状评分显著更低(t=9.49、8.34、10.12,P<0.05);观察组治疗后TGF-β、IL-6、 TNF-α水平显著更低(t=7.85、11.04、7.92,P<0.05);观察组治疗后RTOG、KPS评分明显更优对照组(t=11.13、7.09,P<0.05);观察组临床有效率为90.32%,显著高于对照组的67.74%(χ;=4.77,P<0.05)。结论:沙参麦冬汤治疗RP疗效良好,对缓解临床症状、降低TGF-β、IL-6、 TNF-α水平具有显著效果,有助于改善肺炎分级和身体状态。Objective:To investigate the clinical efficacy of Shashen Maidong decoction combined with western medicine in the treatment of radiation pneumonia (RP).Methods:62 cases of RP patients were divided into control group and observation group according to random number table method,31 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Shashen Maidong decoction on the basis of their treatment.7d was a course of treatment.After 2 courses of treatment,the clinical efficacy,main symptom scores,laboratory indicators,RTOG grading scores and KPS scores were compared between the two groups.Results:Compared with the control group,the main symptom scores of dry expectoration,dry throat and mouth,shortness of breath and fatigue in the observation group were significantly lower after treatment (t=9.49,8.34,10.12,P<0.05);TGF-β in the observation group after treatment,IL-6 and TNF-α levels were significantly lower (t=7.85,11.04,7.92,P<0.05);RTOG and KPS scores in the observation group after treatment were significantly better than those in the control group (t=11.13,7.09,P<0.05);The clinical effective rate in the observation group was 90.32%,which was significantly higher than that in the control group (67.74%) (χ~2=4.77,P<0.05).Conclusion:Shashen Maidong Decoction has a good curative effect in the treatment of RP,and has a significant effect on relieving clinical symptoms,reducing the levels of TGF-β,IL-6 and TNF-α,and helps to improve the classification of pneumonia and physical status.
