穴位埋线调节胃肠激素干预糖尿病前期的临床观察

摘　　要：目的:观察穴位埋线干预糖尿病前期的有效性、安全性及对胃肠激素调节的影响。方法:选择符合糖尿病前期的60名受试者按照随机对照试验方法,分为治疗组和对照组各30例,对照组给予生活方式干预,治疗组在生活方式干预的基础上配合穴位埋线治疗,15d穴位埋线1次,共完成6次穴位埋线。比较2组治疗前后空腹血糖(FPG)、餐后2h血糖(2hPG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)、体重、腰围、体重指数(BMI)的变化,并检测生长激素释放肽(Ghrelin)水平,观察并记录中医症状积分及治疗组治疗期间发生的不良反应。结果:2组治疗后FBG、2hPG、FINS、HbA1c水平下降,体重、腰围、BMI及中医症状积分均有下降,且治疗组明显低于对照组,差异有统计学意义;2组治疗后Ghrelin水平较治疗前升高,且治疗组较对照组明显;观察埋线后发生6种术后反应,包括饮食抑制、全身肌肉酸痛和局部疼痛、血肿和淤青,均未发生发热和局部感染情况。结论:穴位埋线干预糖尿病前期效果明显,可有效降低体重、腰围、体重指数及血糖相关生化指标,这可能是与调节体内生长激素释放肽水平有关,且无明显不良反应,能有效改善中医临床症状,值得临床研究和推广。Objective:To observe the efficacy and safety of acupoint catgut embedding in prediabetes and its effect on the regulation of gastrointestinal hormones.Methods:60 subjects with pre-diabetes were selected and divided into a treatment group and a control group with 30 cases in each according to the method of randomized controlled trial.The control group was given lifestyle intervention,and the treatment group was given acupoint catgut embedding therapy based on lifestyle intervention.,15d acupoint catgut embedding once,a total of 6 times acupoint catgut embedding was completed.The changes of fasting blood glucose (FPG),2h postprandial blood glucose (2hPG),fasting insulin (FINS),glycosylated hemoglobin (HbA1c),body weight,waist circumference and body mass index (BMI) between the two groups before and after treatment were compared,and growth hormone releasing peptide (Ghrelin) level,observed and recorded TCM symptom scores and adverse reactions in the treatment group during treatment.Results:After treatment,the levels of FBG,2hPG,FINS and HbA1c in the two groups decreased,and the body weight,waist circumference,BMI and TCM symptom scores all decreased,and the treatment group was significantly lower than the control group,and the difference was statistically significant;the Ghrelin levels in the two groups after treatment Compared with before treatment,the treatment group was significantly higher than the control group;6 postoperative reactions occurred after thread embedding,including diet inhibition,general muscle aches and local pain,hematoma and bruising,and no fever and local infection occurred.Conclusion:Acupoint catgut embedding has obvious effect on prediabetes,and can effectively reduce body weight,waist circumference,body mass index and blood sugar-related biochemical indicators,which may be related to regulating the level of ghrelin in the body,and there is no obvious adverse reaction,which can effectively improve the clinical practice of traditional Chinese medicine.Symptoms are worthy of clinical research
