自拟益气泄浊汤治疗糖尿病肾病疗效及对患者氧化应激状态及免疫功能的影响

摘　　要：目的:探究糖尿病肾病患者应用自拟益气泄浊汤的效果及对免疫功能、氧化应激状态的影响。方法:选取95例糖尿病肾病维持性血液透析患者作为研究对象,采用随机数字表法进行分组,对照组47例采用连续性血液净化治疗,观察组48例增加自拟降浊固本汤治疗,连续治疗15d后比较2组患者主要症状的评分,氧化应激状态及免疫功能指标。结果:观察组谷胱甘肽过氧化物酶(Glutathione peroxidase,GSH-Px)及超氧化物歧化酶(Superoxide Dismutase,SOD)水平低于对照组,晚期氧化蛋白产物(Advanced oxidation protein products,AOPP)水平高于对照组;观察组CD4+及CD4+/CD8+水平高于对照组,CD8+水平低于对照组;观察组患者肢体浮肿、倦怠乏力、气短、纳呆腹胀、夜尿清长、腰膝酸软等症状评分均低于对照组,差异均有统计学意义。观察组发生恶心、腹泻、腹痛例数与对照组比较,差异无统计学意义。结论:加用自拟益气泄浊汤辅助治疗糖尿病肾病患者效果显著,可明显改善患者氧化应激状态,增强免疫功能,减轻临床主要症状,且安全性高。Objective:To explore the effect of self-made Yiqi Xiezhuo Decoction and its effect on immune function and oxidative stress state in patients with diabetic nephropathy.Methods:95 patients with diabetic nephropathy on maintenance hemodialysis were selected as the research objects,and were divided into groups by random number table method.47 patients in the control group were treated with continuous blood purification,and 48 patients in the observation group were treated with self-made Jiangzhuo Guben decoction.After 15 days of treatment,the scores of main symptoms,oxidative stress and immune function indexes were compared between the two groups.Results:The levels of glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD) in the observation group were lower than those in the control group,and the levels of advanced oxidation protein products (AOPP) were lower than those in the control group.The level of CD4+ and CD4+/CD8+ in the observation group was higher than that in the control group,and the level of CD8+ in the observation group was lower than that in the control group.The symptom scores were lower than those in the control group,and the differences were statistically significant.There was no significant difference in the number of cases of nausea,diarrhea and abdominal pain between the observation group and the control group.Conclusion:The addition of self-made Yiqi Xiezhuo decoction in the adjuvant treatment of diabetic nephropathy has a significant effect,can significantly improve the oxidative stress state,enhance the immune function,relieve the main clinical symptoms,and has high safety.
