摘  要:目的 探讨2-甲氧雌二醇(2-ME)对胎儿生长受限(FGR)胎盘血管内皮功能的影响。方法 选择2019年5月至2020年12月期间南通大学附属妇幼保健院收治的胎儿生长受限(FGR)孕妇66例及健康正常孕检孕妇50例为研究对象,将其分为研究组和健康组。收集并测定研究组与健康组孕妇外周血和脐血2-ME及胎盘组织儿茶酚氧位甲基转移酶(COMT)水平,分析外周血2-ME与脐血2-ME、胎盘组织COMT水平的相关性。将应用不同浓度2-ME(5、10、15μmol/L)处理人胎盘微血管内皮细胞(hPMEC)作为2-ME处理组,处理2d后分别应用CCK-8、Transwell细胞小室及Matrigel胶模型测定细胞增殖、迁移、小管形成,空白对照组则采用等量培养基处理,对测定结果进行统计分析。结果 研究组外周血2-ME水平显著低于健康组(t=11.911,P<0.05),研究组与健康组脐血2-ME水平比较差异无统计学意义(P>0.05);研究组胎盘组织中COMT水平显著低于健康组(t=10.416,P<0.05)。经Pearson相关性分析显示,外周血2-ME与脐血2-ME、胎盘组织COMT均呈正相关(r分别为0.313、0.628,P<0.05),脐血2-ME与胎盘组织COMT呈正相关(r=0.415,P<0.05)。不同浓度2-ME处理组(5、10、15μmol/L)的hPMEC在24h和48h的细胞抑制率均显著低于空白对照组(F值分别为18.907、18.593,P<0.05),且随着2-ME处理浓度的增加,24h和48h的细胞抑制率均逐渐下降(t=3.166~10.075,P<0.05);不同浓度2-ME处理组(5、10、15μmol/L)的hPMEC迁移细胞数目和血管内皮细胞小管形成度均显著高于空白对照组(F值分别为25.751、42.215,P<0.05),且随着2-ME处理浓度的增加,迁移细胞数目和血管内皮细胞小管形成度均逐渐增加(t=3.607~15.066,P<0.05)。结论 FGR孕妇的2-ME、COMT水平表达异常,且2-ME可以促进胎盘血管内皮细胞增殖、迁移,介导血管形成。Objective To investigate the influence of 2-methoxyestradiol(2-ME) on placental vascular endothelial function in pregnant women with fetal growth restriction(FGR).Methods A total of 66 pregnant women with FGR admitted to Affiliated Maternity and Child Health Care Hospital of Nantong University between May 2019 and December 2020(study group),and 50 healthy pregnant women(healthy group) were selected as the research subjects.The levels of 2-ME in peripheral blood and cord blood, and the level of catechol-O-methyltransferase(COMT) in placental tissues of the two groups were measured, and their correlation was analyzed.Human placental microvascular endothelial cells(hPMEC) were treated with different concentrations of 2-ME(5 μmol/L,10 μmol/L,15 μmol/L),and taken as 2-ME treatment group.2 days later, CCK-8,Transwell chamber coculture and matrigel were used to measure the cell proliferation, migration, and tubule formation.The blank control group was treated with the same amount of medium, and the measurement results were statistically analyzed.Results Peripheral blood 2-ME level in the study group was significantly lower than that in the healthy group(t=11.911,P<0.05),while cord blood 2-ME showed no statistically significant difference between the two groups(P>0.05).The COMT level in placental tissue of the study group was significantly lower than that in the healthy group(t=10.416,P<0.05).Pearson correlation analysis found that peripheral blood 2-ME was positively correlated with cord blood 2-ME and placental tissue COMT(r=0.313 and 0.628,respectively, P<0.05).Cord blood 2-ME was positively correlated with placental tissue COMT(r=0.415,P<0.05).The 24 h and 48 h inhibition rates of hPMEC in 2-ME(5,10 and 15μmol/L) treatment groups were significantly lower than those in the blank control group(F=18.907 and 18.593,respectively, P<0.05).As the concentration of 2-ME increased, the inhibition rates decreased(t=3.166-10.075,P<0.05).The migratory hPMEC count and endothelial cell tubule formation in the 2-ME(5,10 and 15μ
