摘  要:目的探讨支气管哮喘患儿肺功能、免疫功能、血清骨膜蛋白(Periostin)水平、白介素-13(IL-13)水平、微小RNA-155(miR-155)水平、哮喘相关炎症因子水平及相关性。方法选择2019年1月至2020年1月在上海交通大学医学院附属第九人民医院就诊的支气管哮喘患儿127例为研究对象(研究组);另选择同时段在该院接受健康体检的儿童127例为健康志愿者(对照组)。监测两组儿童肺功能的变化,测定Periostin、IL-13、miR-155、转化生长因子-β1(TGF-β1)、γ-干扰素(IFN-γ)、缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)、T淋巴细胞水平;分析Periostin、IL-13、miR-155在支气管哮喘患儿血清中的表达水平及相关性。结果相比健康志愿者,支气管哮喘患儿的第1秒用力呼气容积(FEV1)、第1秒用力呼气量占用力肺活量比值(FEV1/FVC%)、最大呼气50%流量(MEF50)水平均下降(t值分别为10.080、9.263、19.840,P<0.05)。相比健康志愿者、哮喘缓解期患儿,哮喘急性期患儿Periostin、IL-13、miR-155水平均上升(F值分别为28.234、67.842、109.352,P<0.05)。与健康志愿者、哮喘缓解期患儿相比,哮喘急性期患儿TGF-β1、HIF-1α、VEGF水平均上升,IFN-γ、CD3^(+)、CD4^(+)、CD8^(+)水平均下降(F值分别为59.621、54.096、55.296、30.623、25.835、38.553、15.482,P<0.05)。Periostin与IL-13呈正相关(r=0.403,P=0.001),Periostin与miR-155呈正相关(r=0.655,P=0.001),IL-13与miR-155呈正相关(r=0.482,P=0.001)。结论检测Periostin、IL-13、miR-155水平对支气管哮喘患儿的诊断及预后具有重要意义,且Periostin、IL-13、miR-155水平均与支气管哮喘具有一定的相关性。Objective To investigate the pulmonary function,immune function,levels of serum Periostin,interleukin-13(IL-13),mi-croRNA-155(miR-155),inflammatory factors related to asthma and their correlations in children with bronchial asthma.Methods A total of 127children with bronchial asthma,who were treated in the Ninth People′s Hospital Shanghai Jiao Tong University School Of Medicine from January 2019to January 2020,were selected as the study subjects(study group).In addition,127children who received physical examination in this hospital at the same time were selected as the healthy volunteers(control group).The changes of pulmonary function in the two groups were monitored.The levels of Periostin,IL-13,miR-155,transforming growth factor-β1(TGF-β1),interferonγ(IFN-γ),hypoxia-inducible factor-1α(HIF-1α),vascular endothelial growth factor(VEGF),and T lympho-cyte were evaluated.The expression levels and correlations of Periostin,IL-13and miR-155in children with bronchial asthma were analyzed.Results Compared with healthy volunteers,the forced expiratory volume in 1second(FEV1),ratio of forced expiratory volume in 1second to forced vital capacity(FEV1/FVC%),maximal expiratory flow at 50%(MEF50)levels in children with bronchial asthma were lower(t=10.080,9.263and 19.840,respectively,P<0.05).Compared with healthy volunteers and chil-dren with asthma in remission stage,the levels of Periostin,IL-13and miR-155in children with asthma in acute stage were increased(F=28.234,67.842and 109.352,respectively,P<0.05).Compared with healthy volunteers and children with asthma in remis-sion stage,the levels of TGF-β1,HIF-1αand VEGF in children with asthma in acute stage increased,and the levels of IFN-γ,CD3^(+),CD4^(+)and CD8^(+)decreased(F=59.621,54.096,55.296,30.623,25.835,38.553and 15.482,respectively,P<0.05).Peri-ostin was positively correlated with IL-13(r=0.403,P=0.001),Periostin was positively correlated with miR-155(r=0.655,P=0.001),and IL-13was positively correlated with miR-155(r=0.482,P=0.001).Conclusion The detecti
