摘　　要：目的通过分析2015—2020年榆林市HIV抗体筛查阳性样本的确证实验结果,了解榆林市HIV流行特征及规律,为制定HIV相关政策提供依据。方法对各HIV筛查实验室送检的抗体初筛阳性样本1157份用免疫印迹法(Western blotting,WB)进行确证实验,用SPSS 17.0软件进行统计学分析。结果2015—2020年榆林市送检的1157份初筛阳性样本中,确证阳性样本834份,占72.08%,主要来源为医疗机构、血液中心、疾控机构及监管场所,差异均有统计学意义(χ^(2)=388.611、45.870、347.271,P<0.05);确证阳性样本中,男性、女性样本阳性率分别为80.82%、19.18%,男女比例为4.2∶1;20~40岁组阳性率为43.05%,远高于其他年龄组;已婚人群阳性率为72.06%,明显高于未婚人群;文化程度以小学、初中为主,分别占24.22%、24.46%,性别、年龄、婚姻状况、文化程度差异均有统计学意义(χ^(2)=82.779、210.669、441.902、86.079,P<0.01)。2015—2020年确证阳性病例成逐年增加趋势。结论2015—2020年榆林市艾滋病疫情不容乐观,要广泛开展艾滋病的宣传服务,提高筛查实验室工作人员的检测能力,并做好相关的质量控制,尽早发现阳性病例,遏制艾滋病蔓延。Objective To understand HIV epidemiological characteristics and laws in Yulin by analyzing the results of confirmatory tests for HIV antibody screening positive samples in Yulin from 2015 to 2020,and to provide the basis for formulating HIV related policies.Methods Totally 1157 samples preliminarily screened to be positive by HIV antibody screening laboratories were subject to confirmatory tests with Western blotting(WB).The software SPSS 17.0 was applied for statistical analysis.Results Totally 834 samples out of 1157 samples preliminarily screened to be positive from 2015 to 2020 were confirmed to be positive,accounting for 72.08%,and the samples were mainly from medical institutions,blood centers,disease control institutions and regulatory places,and the differences were statistically significant(χ^(2)=388.611,45.870,347.271,P<0.05).In the confirmed positive samples,the positive rate of man and woman′s samples were 80.82%and 19.18%,respectively,and the male-to-female ratio was 4.2∶1 approximately;the positive rate of the proportion aged 20-40 years old group was 43.05%,being much higher than other age groups;the positive rate of married population was 72.06%,being significantly higher than that of unmarried population.The education level was dominated by primary school and junior middle school with a percent of 24.22%and 24.46%respectively.The differences in the sex,age,marital status and education level were statistically significant(χ^(2)=82.779,210.669,441.902,86.079,P<0.01).The confirmed positive cases showed incremental trend year after year from 2015 to 2020.Conclusion The epidemic situation of AIDS in Yulin from 2015 to 2020 is not optimistic,it is necessary to conduct AIDS-related publicity service extensively,and strengthen the test abilities of the screening laboratory workers,properly implement the quality control,find the positive cases at earliest convenience,and curb the spread of AIDS.
