摘 要:目的传统的结核分枝杆菌鉴定方法周期时间长、敏感性较差,往往延误患者的诊断和治疗,随着分子生物学的飞速发展,为了可以快速诊断结核分枝杆菌,本研究利用重组酶聚合酶扩增(Recombinase Polymerase Amplification,RPA)技术建立快速的结核分枝杆菌检测方法,为结核病患者的诊断和治疗赢得宝贵时间。方法应用RPA技术对疑似肺结核患者痰标本中的特异性核苷酸片段进行检测,利用卫生统计学检验RPA技术与传统诊断方法有无差异。结果80例疑似肺结核患者的痰标本中检测出70例阳性,有10例排除结核病。80例疑似肺结核患者的痰标本中,RPA技术检测阳性率为87.50%,痰涂片检测阳性率为56.25%,痰培养检测阳性率为75.00%,RPA技术检测阳性率较高,与痰涂片、痰培养比较,差异均有统计学意义(χ^(2)=25.00,P<0.01;χ^(2)=10.00,P<0.01)。结论RPA技术的成功建立对结核病的快速诊断有着重要意义。Objective To earn valuable time for the diagnosis and treatment of patients with tuberculosis arising from traditional identification methods of Mycobacterium tuberculosis with long cycle and poor sensitivity,delayed diagnosis&treatment of patients frequently occurring,rapid detection method for Mycobacterium tuberculosis was established in the present study by Recombinase Polymerase Amplification(RPA)technology with the rapid development of molecular biology to quickly diagnose the Mycobacterium tuberculosis.Methods RPA technology was applied to detect specific nucleotide fragments in sputum samples of suspected cases,and health statistics was used to test the difference between RPA technology and traditional diagnostic method.Results The 70 sputum samples out of 80 patients with suspected pulmonary tuberculosis were tested to be positive,and 10 cases were excluded from tuberculosis.In the sputum samples of 80 patients with suspected pulmonary tuberculosis,the tested positive rate with RPA technology was 87.50%,the positive rate tested with sputum smear was 56.25%,the positive rate tested with sputum culture was 75.00%,the positive rate tested with RPA technology was high,and all the differences were statistically significant compared with the sputum smear and sputum culture(χ^(2)=25.00,P<0.01;χ^(2)=10.00,P<0.01).Conclusion The successful establishment of RPA technology is of great significance for the rapid diagnosis of tuberculosis.
