摘 要:炎症与肿瘤关系密切,肿瘤坏死因子α(TNF-α)是参与炎症的主要炎性细胞因子之一,在各种慢性炎症性疾病及肿瘤环境中高表达。TNF-α可诱导其他炎性因子释放放大炎症反应,还可通过释放氧氮介质直接诱导上皮细胞癌变促进肿瘤发生。此外TNF-α还可以通过激活NF-κB等炎症通路促进肿瘤细胞增殖,TNF-α上调血管内皮生长因子表达,促进肿瘤组织微血管生成,促进肿瘤的发展。TNF-α可通过激活NF-κBp52抑制肿瘤细胞凋亡,且可抑制免疫细胞的抗肿瘤活性从而促进肿瘤细胞的免疫逃逸。基于TNF-α在慢性炎症相关性肿瘤的发生、发展、转移及免疫逃逸等多方面均发挥重要作用,TNF-α可成为抗慢性炎症相关性肿瘤精准医疗的潜力靶点,从多个环节、多条途径来发挥抗肿瘤的作用,目前有多个具有TNF-α抑制药作用的天然产物化合物处于试验阶段。本文重点探讨慢性炎症与肿瘤发生中TNF-α的作用,以期为临床上常见的TNF-α相关慢性炎症所致肿瘤的预防及治疗提供理论基础。Inflammation is considered to be closely related to tumor.Tumor necrosis factor-alpha(TNF-α)as one of the main inflammatory cytokines involved in inflammation it is highly expressed in various chronic inflammatory diseases and tumor environments.TNF-αcan induce the release of other inflammatory factors to amplify the inflammatory response,directly induce epithelial cell carcinogenesis and promote tumorigenesis by releasing oxygen and nitrogen mediators.TNF-αpromotes tumor cell proliferation by activated inflammatory pathways such as NF-κB pathway and others.TNF-αup regulates the expression of vascular endothelial growth factor,increases the angiogenesis of tumor tissue,so it can promote the development of tumor.TNF-αinhibits tumor cell apoptosis by activating NF-κBp52,and reduces the antitumor activity of immune cells,both of them make tumor cells immune escape.Because TNF-αplays an important role in the occurrence,development,metastasis and immune escape of chronic inflammation associated tumor,it can become a potential target for precision medicine against chronic inflammation related tumors,and play an anti-tumor role from multiple nodes and pathways.At present,many natural product compounds with TNF-αinhibitors are under research and development.In this paper we focus on the role of TNF-αin chronic inflammation and tumorigenesis expected to provide a theoretical basis for the prevention and treatment of tumors caused by chronic TNF-αrelated chronic inflammation.
