玉屏风颗粒联合针刺蝶腭神经节治疗中重度持续变应性鼻炎的临床疗效观察

摘　　要：目的观察玉屏风颗粒联合针刺蝶腭神经节对中重度持续性变应性鼻炎(AR)的治疗效果,并探讨对免疫失衡的调节作用。方法选取海南省中医院2019年6月至2020年12月收治的中重度持续性AR患者165例,随机分为联合组、中药组、西药组,每组55例,联合组给予玉屏风颗粒联合针刺蝶腭神经节治疗,中药组单用玉屏风颗粒治疗,西药组给予丙酸氟替卡松鼻喷雾剂治疗,三组疗程均为3周,随访6个月。观察三组患者治疗前、后及随访结束时鼻部症状评分和体征评分、临床疗效及疾病复发率,并比较三组患者治疗前、后及随访结束时血清IL-4、IFN-γ、IgE水平。结果与治疗前比较,联合组治疗后、随访结束时症状评分和体征评分均降低,且低于同期中药组和西药组(P<0.05),治疗后联合组总有效率为94.5%,高于中药组的74.5%和西药组的69.1%(P<0.05),随访期间联合组疾病复发率为13.5%,低于中药组的36.6%和西药组的86.8%(P<0.05),联合组治疗后及随访结束时血清IL-4、IgE水平均低于中药组和西药组,IFN-γ水平高于中药组和西药组(P<0.05)。结论玉屏风颗粒联合针刺蝶腭神经节治疗中重度持续性AR的近、远期疗效显著,能有效减少疾病复发,其作用机制可能与调节机体免疫失衡有关。Objective To observe the clinical efficacy of Yupingfeng granule combined with acupuncture of sphenopalatine ganglion in treatment of moderate and severe persistent allergic rhinitis(AR)and its regulatory effect on immune imbalance.Methods 165 cases of patients with moderate and severe persistent AR who treated in our hospital from June 2019 to December 2020 are randomly divided into combination group,traditional Chinese medicine group and western medicine group,55 cases in each group.The combination group are treated with Yupingfeng granule combined with acupuncture of sphenopalatine ganglion.The traditional Chinese medicine group are treated with Yupingfeng granule.The western medicine group are treated with fluticasone propionate.Three groups are treated for 3 weeks and followed up for 6 months.symptom and sign scores,clinical efficacy and disease recurrence rate in three groups are observed before treatment,after treatment and the end of follow-up.And the levels of IL-4,IFN-γand IgE are compared between the three groups before treatment,after treatment and the end of follow-up.Results Compared with before treatment,symptom and sign scores of the combination group are lower at the end of treatment and follow-up,and lower than those of the traditional Chinese medicine group and the western medicine group at the same period(P<0.05).The total efficiency of the combination group after treatment is 94.5%,which is higher than that of the traditional Chinese medicine group(74.5%)and the western medicine group(69.1%)(P<0.05).The disease recurrence rate of the combination group is 13.5%,which is lower than that of the traditional Chinese medicine group(36.6%)and the western medicine group(86.8%)during the follow-up(P<0.05).The levels of serum IL-4 and IgE in the combination group are lower than those in the traditional Chinese medicine group and the Western medicine group,while the levels of serum IFN-γin the combined group are higher than those in the traditional Chinese medicine group and the Western medicine group
