摘 要:目的探讨三维超短回波时间(three-dimensions ultra short echo time,3D UTE)序列和三维梯度回波(three-dimensions gradient echo,3D GRE)序列对早产儿肺部磁共振成像的可行性,优化磁共振序列扫描参数。材料与方法前瞻性纳入于2021年3月至7月在我院行肺部MR检查的早产儿95例,男婴50例,女婴45例,年龄1天~1岁10个月,中位年龄5个月。所有志愿者均在联影3.0 T儿科专用磁共振设备(UIH uMR Alpha)上进行肺部磁共振检查,扫描序列包括3D UTE和3D GRE序列。各序列分别采用不同扫描参数进行序列调试。采用随机分配的方法将3D UTE序列按照不同扫描参数组别将所有受试者分为组一(18例)、组二(17例)、组三(19例)、组四(21例)和组五(20例)等五个组别。3D GRE序列按照不同扫描参数组别将所有受试者分为组Ⅰ(24例)、组Ⅱ(22例)、组Ⅲ(23例)和组Ⅳ(26例)等四个组别。两名放射科医师采用客观评价法对不同扫描参数时图像质量进行评价,客观评价指标包括信噪比(signal noise ratio,SNR)和对比噪声比(contrast noise ratio,CNR),并进一步对不同组间扫描时间进行比较。结果3D UTE和3D GRE序列不同扫描参数时肺部图像SNR和CNR差异有统计学意义(P<0.05)。在3D UTE序列中,组一、二、三分别与组四和组五图像的SNR和CNR差异有统计学意义(P<0.05),而组四和组五图像质量差异无统计学意义(P>0.05)。同时,组四较组一、二、三扫描时间分别减少77.95%、75.55%和6.67%,组五较组一、二扫描时间分别减少76.06%和73.45%。在3D GRE序列中,组Ⅰ、Ⅱ分别与组Ⅲ、Ⅳ图像的SNR和CNR差异有统计学意义(P<0.05),而组Ⅲ与组Ⅳ图像质量差异无统计学意义(P>0.05)。同时,组Ⅲ较组Ⅰ、Ⅱ扫描时间分别减少70.80%和24.24%,组Ⅳ较组Ⅰ扫描时间减少40.71%,而图像质量却显著提高。结论3D UTE和3D GRE序列对早产儿肺部进行磁共振成像是可行的,3D UTE序列推荐扫描参数为组四和Objective:To explore the feasibility of three-dimensions ultra short echo time(3D UTE)sequence and three-dimensions gradient echo(3D GRE)sequence for magnetic resonance imaging of preterm infants’lungs,and to optimize magnetic resonance sequence scanning parameters.Materials and Methods:Prospectively enrolled 95 preterm infants,50 male infants and 45 female infants who underwent pulmonary MR examinations in our hospital from March to July 2021,aged from 1 day to 1 year and 10 months,and the median age was 5 months.All volunteers performed lung magnetic resonance examinations on the United Imaging 3.0 T pediatric specific magnetic resonance equipment(UIH uMR Alpha).The scanning sequence included 3D UTE and 3D GRE sequences.Each sequence uses different scanning parameters for sequence debugging.According to different scanning parameters,all subjects were randomly divided into five groups in 3D UTE sequence:Group 1(18 cases),Group 2(17 cases),Group 3(19 cases),Group 4(21 cases)and Group 5(20 cases).3D GRE sequence were randomly divided into four groups:GroupⅠ(24 cases),GroupⅡ(22 cases),GroupⅢ(23 cases)and GroupⅣ(26 cases).Two radiologists evaluated the image quality at different scanning parameters using objective evaluation methods.The objective evaluation indicators include signal noise ratio(SNR)and contrast noise ratio(CNR).Scanning time among different groups were further compared.Results:There were statistically significant differences(P<0.05)in SNR and CNR of lung images at different scanning parameters for the 3D UTE and 3D GRE sequences.In the 3D UTE sequence,the SNR and CNR of Group 1,2,and 3 were significantly different from those of Group 4 and 5,respectively(P<0.05),while there was no significant difference in image quality between Group 4 and 5(P<0.05).Meanwhile,the scanning time was reduced by 77.95%,75.55%and 6.67%in Group 4 compared with Group1,2,and 3,and 76.06%and 73.45%in Group 5 compared with Groups 1 and 2,respectively.In the 3D GRE sequence,the SNR and CNR of GroupⅠandⅡwere stati
