摘　　要：肥胖是21世纪最大的医疗保健挑战之一,研究表明,肥胖以及由此引发的糖尿病、心血管疾病、代谢综合症等直接或间接引起睾丸激素水平下降、精子生成受损、性功能障碍,增加生殖相关疾病风险,引起全身及生殖器局部炎症、精神心理障碍等,导致精子质量降低、精卵相遇及结合障碍,从而对育龄男性的生殖健康产生严重影响。肥胖所引起的精液质量低下、精子DNA损伤是导致受精障碍、妊娠丢失、体外受精囊胚形成率和着床率降低等不良后果的重要因素。然而相关影响及机制目前仍未完全阐明,现结合临床体会并就目前的观点予以综述,以期对临床提供指导。Obesity is one of the greatest challenges of health care in the 21st century,and researches show that obesity and the diseases resulting from obesity such as diabetes,cardiovascular disease,metabolic syndrome and so on directly or indirectly cause the drop of testosterone levels,impairment of sperm production,sexual dysfunction,increase of the risk related to reproductive disease,the whole body and genitals local inflammation,mental disorders,etc.,and lead to the reduction of sperm quality and the obstacles sperm-egg meeting and binding.This has a serious impact on the reproductive health of men of childbearing age.Low semen quality and sperm DNA damage caused by obesity are important factors leading to adverse consequences such as fertilization disorder,pregnancy loss,reduction of blastocyst formation rate and implantation rate in vitro fertilization.However,the relevant effects and mechanisms have not been fully elucidated.This paper reviews the current views based on clinical experience in order to provide guidance for clinical practice.
