2015-2019年河南省洛阳市含无细胞百白破成分联合疫苗预防接种安全性分析

摘　　要：目的分析河南省洛阳市无细胞百白破联合疫苗(DTaP)、无细胞百白破b型流感嗜血杆菌(结合)联合疫苗(DTaP-Hib)、无细胞百白破灭活脊灰和b型流感嗜血杆菌(结合)联合疫苗(DTaP-IPV/Hib)的预防接种安全性。方法通过中国免疫规划信息管理系统收集2015-2019年洛阳市报告的DTaP、DTaP-Hib和DTaP-IPV/Hib疑似预防接种异常反应(AEFI)个案数据,采用描述流行病学方法进行分析。结果2015-2019年洛阳市含DTaP成分疫苗报告AEFI个案2924例,报告发生率160.06/10万剂,其中DTaP为154.82/10万剂、DTaP-Hib为239.48/10万剂、DTaP-IPV/Hib为234.20/10万剂,3类疫苗AEFI报告发生率差异具有统计学意义(χ^(2)=46.283,P<0.05);男女比例1.34∶1,3~24月龄2728例(93.30%),接种后1 d内报告2724例(93.16%);一般反应报告2890例(98.84%),多以发热、红肿、硬结为主,异常反应报告29例(0.99%),主要临床诊断为过敏性反应疾病、局部化脓性感染、无菌性脓肿等。结论洛阳市上市使用的含无细胞百白破成分疫苗AEFI报告发生率较低,多为一般反应,严重异常反应罕见,疫苗安全性良好。Objective To analyse the safety of diphtheria,tetanus and acellular pertussis(DTaP)containing combined vaccines in Luoyang.Methods The adverse events following immunization(AEFI)cases of DTaP、Diphtheria,Tetanus and Acellular Pertussis and Haemophilus Influenzae Type b Combined Vaccine(DTaP-Hib),and Diphtheria,Tetanus and Acellular Pertussis poliomyelitis(inactivated)and Haemophilus Influenzae Type b Combined Vaccine(DTaP-IPV-Hib)in Luoyang from 2015 to2019 were obtained through Chinese Immunization Information Management System for descriptive epidemiological analyses.Results From 2015 to 2019,2924 cases were reported with the incidence of 160.06 per 100000 doses.The incidence of 3 types of vaccines was 154.50 per 100000 doses of DTaP,239.48 per 100000 doses of DTaP-Hib and 234.20 per 100000 doses of DTaP-IPV/Hib respectively,with statistical significance(χ^(2)=46.283,P<0.05).The ratio of male to female was 1.34:1,2728cases(93.30%)were occurred 3-24 months old,2247 cases(93.16%)were reported within 1 day after immunization;2890cases were general reactions(98.84%),most of which were fever,redness and scleroma.29 cases were abnormal reactions(0.99%),the main clinical diagnosis was allergic reaction disease,local suppurative infection,aseptic abscess.Conclusion The main adverse reactions of diphtheria,tetanus and acellular pertussis containing combined vaccines in Luoyang were general reactions,with low incidence,serious reactions rare and good safety.
