摘　　要：近年来许多研究倾向于特应性皮炎(atopic dermatitis,AD)是一种系统性疾病,不仅会伴发其他特应性疾病,还与很多非特应性疾病相关。在临床实践中,越来越多的医护工作者发现部分AD患者常伴发自身免疫性疾病,且自身免疫性疾病会加重AD患者疾病负担,研究两者相关性及其机制至关重要。研究已发现两者关系密切且机制复杂,AD会增加患自身免疫性疾病的风险,自身免疫性疾病也是患AD的危险因素,尤其是累及皮肤及肠黏膜的自身免疫性疾病;但AD与自身免疫性甲状腺疾病及风湿性自身免疫病的相关性存在矛盾观点。探讨两者相关性及可能机制有助于提高对AD共病的认识,筛选患自身免疫性疾病的高危AD亚型并进行早期干预,对改善患者生活质量及减轻生活负担具有重要意义。In recent years,many studies have favored atopic dermatitis(AD)as a systemic disease that is not only associated with other atopic diseases,but also associated with many non-atopic diseases.In clinical practice,more and more medical workers find that some AD patients are often associated with autoimmune diseases,and autoimmune diseases aggravate the disease burden of AD patients.It is essential to study the correlation between the two and its mechanism.It has been found that there is a close relationship between the two and the mechanism is complex.AD will increase the risk of autoimmune diseases.Autoimmune diseases are also risk factors of AD,especially autoimmune diseases involving the skin and intestinal mucosa;however,there are contradictory views on the correlation between AD and autoimmune thyroid diseases and rheumatic autoimmune diseases.Exploring the correlation and possible mechanism between the two is helpful to improve the understanding of AD comorbidity,screen AD subtypes with autoimmune diseases and perform early intervention,which is important to improve the quality of life of patients and reduce the burden of life.
